La place de l’imagerie en cancérologiedu tube digestif est capitale, à des degrés diversen fonction du segment digestif, pour le diagnostic positif, lacaractérisation, le bilan d’extension locale et à distanceet enfin la surveillance pendant et après traitements curatifsou palliatifs. Les protocoles de réalisation doivent êtreoptimaux et reproductibles, car toute la stratégie thérapeutiqueen dépend ; le scanner thoraco-abdomino-pelvienest l’examen clé, optimisé et adapté enfonction du segment digestif concerné, nécessitantune bonne distension digestive ; l’interprétationcombine l’analyse des coupes axiales natives et des reformationsmultiplanaires. Dans certains cas particuliers, il est possibled’avoir recours à d’autres méthodesd’imagerie centrées (IRM, échographiede contraste) ou corps entier (IRM corps entier, TEP-CT).
LaTEP-CT-FDG constitue une approche complémentaire de l’approchemorphologique ; en effet, elle permet d’une part d’identifierdes lésions non vues en imagerie conventionnelle — le plussouvent du fait d’un contraste insuffisant avec les tissus environnants — etd’autre part de caractériser des lésionsconnues. Mais cet examen trouve tout son intérêtdans le cadre d’une intégration effective avecles autres techniques d’imagerie.
Radiologues etmédecins nucléaires apportent des informations complémentaires.Chacun doit comprendre les questions et les enjeux des cliniciens,et également connaître les avantages et les limitesde leurs techniques d’imagerie respectives, afin de fournirau sein d’une équipe multidisciplinaire les réponsespertinentes pour l’élaboration d’unestratégie thérapeutique la plus adéquatepossible.

Imagingplays a pivotal role in the management of GI tract cancers for diagnosis,characterization, locoregional staging, metastatic work-up and follow-upduring and after curative or palliative treatment. The imaging protocolsshould be optimized and reproducible because of their impact ontherapy. Thoracic, abdominal and pelvic CT is the cornerstone ofthe imaging work-up, optimized and tailored to the specific GI segmentinvolved, requiring good GI tract distension. Image interpretationof native axial and reformatted multiplanar images is routinelyperformed. In specific cases, additional targeted imaging with USor MRI or whole body imaging with PET/CT or MRI may bevaluable.
PET/CT is a complement to morphologicalimaging. PET allows detection of lesions otherwise undetected onmorphological imaging, usually due to poor contrast with surroundingtissues, and characterization of known lesions. PET/CTis best used as an integral part of a comprehensive imaging work-up.
Radiologistand nuclear medicine specialist provide complementary information.Each must be familiar with the clinical questions at hand and relatedstakes, and advantages and limitations of each modality to optimizetreatment as part of a multidisciplinary management approach.
