Résumé |
Un garçon de 10 ans est traité depuis 3 ans pour un wheezing intermittent avec un syndrome obstructif modéré, inchangé par le traitement bronchodilatateur et les corticoïdes inhalés. L'interrogatoire et l'auscultation révèlent un sifflement en regard du champ pulmonaire droit ; les radiographies pulmonaires montrent une hyperclarté localisée du lobe moyen uniquement sur le cliché en expiration forcée. La fibroscopie bronchique met en évidence un corps étranger au niveau de la bronche lobaire moyenne. Après son extraction au bronchoscope rigide, les symptômes respiratoires disparaîssent, les clichés thoraciques et la spirométrie se normalisent.

Abstract |
A 10-year-old boy with intermittent wheezing and moderate airway obstruction had been treated during the previous 3 years with inhaled beta-2 agonists and inhaled steroids without improvement. Auscultation of the chest revealed wheezing in the right lung. Chest X-ray showed a radiolucent area in the right middle lobe visible on forced expiration. A foreign body, first seen in the middle lobe bronchus on flexible fiberoptic bronchoscopy, was removed with a rigid bronchoscope. The patient's symptoms then disappeared, and repeated chest X-rays and spirometry were normal thereafter.
