Comparative study of atrial vulnerability in patients with unexplained ischemic stroke or lone atrial paroxysmal fibrillation - 01/01/03

Strokes have a high prevalence, with a high rate of recurrence, and about 30-40% remain of unknown cause. Some patients might have asymptomatic paroxysmal atrial fibrillation (AF) which remains the main cause of embolic events. A latent atrial arrhythmogenic substrate may induce recurrent arrhythmias, including functional abnormalities such as nonuniform refractoriness and/or anatomic abnormalities such as atrial septum aneurysm (ASA) and patent foramen ovale (PFO). In 175 patients divided into three groups (Group I: 103 patients with unexplained ischemic stroke, Group II: 48 patients with paroxysmal AF and Group III or control group: 24 patients explored for another cause), such an atrial arrhythmogenic substrate was assessed by electrophysiological study. Groups I and II had a similar high rate of inducible atrial arrhythmias compared to control group III where no arrhythmia was induced. An induced atrial arrhythmia was observed in more than 50% of patients of Group I and in more than 70% of patients of Group II without any significant difference according to age. However, in 26 young patients of Group I who had a transesophageal echocardiography, both a high rate (46%) of ASA and/or PFO and a frequent latent atrial vulnerability (LAV) were observed, compared to older patients where an atrial septum abnormality was observed in only 21% of cases. Thus, among patients with stroke of unknown cause, a high percentage of them might have asymptomatic atrial paroxysmal arrhythmia. The predictive value of the electrophysiological study for spontaneous arrhythmias and recurrence of stroke remains to be demonstrated.

Les accidents vasculaires cérébraux (AVC) ont une prévalence et un taux de récurrence élevés et environ 30-40% restent inexpliqués. Certains patients pourraient avoir une fibrillation auriculaire (FA) paroxystique asymptomatique qui reste la cause principale d'accidents emboliques. Un substrat arrhythmogénique latent peut provoquer des arythmies récurrentes avec des anomalies fonctionnelles telles qu'une hétérogénéité des périodes réfractaires et/ou des anomalies anatomiques comme un anévrysme du septum interauriculaire (ASA) ou un foramen ovale perméable (FOP). Chez 175 patients divisés en 3 groupes (groupe I : 103 patients avec AVC inexpliqué, groupe II : 48 patients avec FA paroxystique, groupe III : groupe contrôle avec 24 patients explorés pour autre cause), un tel substrat pour les arythmies atriales a été recherché par exploration électrophysiologique endocavitaire. Les groupes I et II avaient un taux similaire d'inductibilité d'arythmies auriculaires comparés au groupe III où aucune arythmie n'était induite. L'induction d'une arythmie auriculaire était observée chez plus de 50% des patients du groupe I et chez plus de 70% des patients du groupe II sans différence significative après ajustement sur l'âge. Cependant, chez 26 patients jeunes du groupe I qui avaient eu une échocardiographie transoesophagienne, on observait à la fois un taux élevé d'ASA et/ou de FOP (46%) et une vulnérabilité auriculaire latente par comparaison aux sujets plus agés où une anomalie du septum interauriculaire n'était observée que dans 21% des cas. Ainsi, parmi les patients avec AVC de cause inexpliquée, un pourcentage élevé pourraient avoir des accès asymptomatiques de FA paroxystique. La valeur prédictive de l'étude électrophysiologique de la vulnérabilité auriculaire pour la survenue d'arythmie spontanée ou la récidive d'AVC reste à démontrer.
