Introduction: Our objective was to evaluate the long-term efficacy of AdVance^TMXP male sling in the treatment of post radical prostatectomy urinary incontinence.

Methods: 86 patients underwent male sling surgery at our center between 2010 and 2017. Data were collected in 2022 from electronic medical records, telephone interviews, standardized and validated questionnaires, including the ICIQ-SF, PGI-I, and Pad Test.

Results: Among 86 patients, 16 deceased prior 2022, 16 had no efficacy at the one-month post-operative evaluation, and 4 were lost to follow-up. Adverse events occurred in 25 cases. With a median follow-up of 100 months, efficacy at the end of follow up was observed in 41/86 (48%) patients. Among the 50 patients who had initial postoperative efficacy, 32 (64%) maintained efficacy at the end of follow-up. Loss of efficacy in the remaining 18 patients occurred with a median of 36 months. Quality of life was significantly decreased in the non-effective group of patients. A history of radiotherapy, a history of operated urethral stricture, severe preoperative urinary incontinence, and age ≥70 years were significantly associated with the loss of efficacy in univariate analysis. In multivariate analysis, only severe preoperative urinary incontinence remained significantly associated with loss of efficacy.

Conclusion: With a median follow-up of 100 months, efficacy at the end of follow up was observed in almost half of all patients.

Introduction: Notre objectif était d’évaluer l’efficacité à long terme des bandelettes sous urétrales AdVance^TMXP dans le traitement de l’incontinence urinaire post prostatectomie.

Méthode: 86 patients ont été opérés dans notre centre d’une bandelette sous urétrale entre 2010 et 2017. Les données ont été recueillies en 2022 sur dossier informatique, par entretien téléphonique, envoi de questionnaires standardisés et validés ICIQ-SF, PGI-I et Pad Test.

Résultats: Parmi les 86 patients, 16 sont décédés avant 2022, l’échec d’emblée à 1 mois post opératoire est survenu chez 16 patients, et 4 ont été perdus de vue. 25 patients ont présenté au moins 1 événement indésirable. Avec un suivi médian de 100 mois, l'efficacité à la fin du suivi était estimée à 41/86 (48%) des patients. Parmi les 50 patients qui avaient une efficacité postopératoire initiale, 32 (64%) ont gardé une efficacité à la fin du suivi. La perte d’efficacité de la bandelette sous urétrale survenait après une médiane de 36 mois chez les 18 patients restants. Les 18 patients restants ont perdu leur efficacité après une période médiane de 36 mois. La qualité de vie était significativement diminuée dans le groupe des patients porteur de bandelette non efficace. Les antécédents de radiothérapie, de sténose urétrale opérée, d'incontinence urinaire préopératoire sévère et l'âge ≥70 ans étaient significativement associés à la perte d'efficacité en analyse univariée. En analyse multivariée, seule l'incontinence urinaire préopératoire sévère était significativement associée à la perte d'efficacité.

Conclusion: Avec une médiane d’évaluation de 100 mois, l'efficacité à la fin du suivi a été observée chez près de la moitié des patients.
