T05-O-05 Atypical depression and problems of early diagnosis - 27/06/08

Atypical depressions represented as a sexual dysfunction quite often mislead the patients to seek the consultation other than psychiatric treatment. The problem of adequate diagnosis could be considered as a major problem for treatment of this group. In clinical setting design for providing consultancy mainly avoid the targeted in-depth interviewing and are focused more on symptom based diagnosis. The research aims to identify the cluster of specific items in short self administrated depression questionnaires for atypical depression focusing to differentiate from major depression.
128 cases of atypical depression F32, 8 (male patients seeking the consultation with different forms of sexual dys-function) were investigated by Beck Depression inventory and compared with group of 30 patients with major depression. The item based analysis was used to compare the differences of these groups.
Results show different cluster of items in atypical depression than in controlled group. Atypical depression is mainly represented with items concerning the self esteem and do not put the emphasis on items evaluating the mood changes. At the same time this pattern is maintained for the different age groups, sexual activity and continuous of complains.
Differentiating the particular cluster of items in self administrated depression inventory could lead to the usage of in-depth interview for assessing the state of personality and adequate diagnosis of atypical depression represented in form of sexual complains.
