Cet article propose une réflexion théorico-clinique basée sur notre pratique hospitalière, en examinant la position singulière du psychologue dans le contexte socioculturel togolais, où la souffrance psychique du sujet et de sa famille est accueillie. Dans ce contexte, les spécificités cliniques sont marquées par l’implication de la famille dans la prise en charge hospitalière et par sa participation à l’élaboration culturelle de l’étiologie spirituelle du trouble. Après avoir analysé l’héritage épistémologique de la discipline au Togo et des facteurs culturels qui y sont associés, nous recentrons notre étude sur l’analyse de l’étiologie spirituelle en utilisant des modèles théoriques psychodynamiques qui permettent de penser les intrications complexes entre psychisme et culture, et entre subjectivité et réalité familiale. Notre étude montre comment les étiologies spirituelles « singulières-plurielles », lorsqu’elles sont reconnues et intégrées dans la clinique, permettent de saisir le sens individuel et collectif du symptôme. Pour se faire, nous étudions à travers un cas clinique, comment les étiologies spirituelles sont réaménagées dans les discours familiaux et s’inscrivent dans différents espaces de réalité : intrapsychique, interpsychique et transpsychique.

This article proposes a theoretical-clinical reflection based on our hospital practice by examining the unique position of the psychologist in the Togolese socio-cultural context where the psychic suffering of the subject and his family is received. In this context, the specific clinical features are marked by the involvement of the family in the hospital treatment and their participation in the cultural elaboration of the spiritual aetiology of the disorder. After analysing the epistemological heritage of the discipline in Togo and the cultural factors associated with it, we refocus our study on the analysis of the spiritual aetiology using psychodynamic theoretical models that allow us to think about the complex entanglements between psyche and culture, and between subjectivity and family reality. Our study shows how “singular-plural” spiritual aetiologies, when recognised and integrated into the clinic, enable us to grasp the individual and collective meaning of the symptom. To do this, we use a clinical case to study how spiritual aetiologies are reorganised in family discourse and inscribed in different spaces of reality: intrapsychic, interpsychic and transpsychic.
