Le lymphome primitif de la surrénale est rare. Nous rapportons le cas d'un patient de 31 ans admis pour lombalgies bilatérales, asthénie et insuffisance surrénalienne. Le scanner abdominal a montré deux images surrénaliennes. Une biopsie scanoguidée de la masse était en faveur d'une tumeur endocrinoïde. L'exploration chirurgicale après incision bisouscostale a montré une énorme masse envahissant l'espace rétropéritonéal. La biopsie a révélé un lymphome malin non hodgkinien de phénotype B centrofolliculaire de grade 2. Le scanner thoracique n'a pas montré d'adénopathies médiastinales. La biopsie ostéomédullaire a éliminé un lymphome secondaire. Le malade a été traité par chimiothérapie associée à une radiothérapie, avec un résultat satisfaisant pendant 16 mois.

Primary non-Hodgkin's lymphoma of the adrenal gland is rare. We report the case of a 31-years-old patient hospitalized with asthenia and adrenal insufficiency. The CT scan showed a bilateral adrenal mass. A scano-guided biopsy suspected an endocrinoid tumor. The surgical exploration demonstrated a huge mass invading the retroperitoneal space, and the biopsy concluded to a central follicular phenotype B rmalignant lymphoma with a high rank of malignity. The thoracic CT scan did not show any lymph node. The medullar biopsy eliminated a secondary lymphoma. The patient was treated by chemotherapy and radiotherapy with a good result during 16 months.
