La Haute Autorité de santé a publié en mai 2025 une actualisation de la place des différents examens d’imagerie pour le diagnostic de l’endométriose. Ce document réaffirme le rôle de l’échographie endovaginale comme examen de première intention pour le diagnostic d’endométriose pelvienne. Il propose des recommandations concernant la réalisation technique de l’examen incluant un temps suspubien et un temps endovaginal, et l’interprétation des images. Il décrit les aspects échographiques des lésions d’endométriose ovarienne, superficielle et profonde rapportés dans la littérature. Il rappelle la faible sensibilité de l’échographie pelvienne pour le diagnostic d’endométriose superficielle, sa forte sensibilité et spécificité pour les localisations ovariennes ou rectale moindre pour les localisations sigmoïdiennes, sa faible sensibilité mais forte spécificité pour les localisations latérales. L’intérêt de l’utilisation de comptes rendus standardisés est souligné, permettant d’homogénéiser la description des anomalies, d’en faire une description exhaustive et de contribuer à une meilleure communication multidisciplinaire ainsi qu’à une optimisation de la prise en charge. La Haute Autorité de santé souligne enfin l’importance de la formation du praticien et la nécessité d’une terminologie commune pour améliorer la reproductibilité et la qualité du diagnostic.

In May 2025, the Haute Autorité de santé (the French national authority for health) published an update on the role of different imaging examinations for the diagnosis of endometriosis. This document reaffirms the role of transvaginal ultrasound as the first-line examination for the diagnosis of pelvic endometriosis. It proposes recommendations regarding the technical aspects of the examination, including a suprapubic and a transvaginal phase, and the interpretation of the images. It describes the ultrasound features of ovarian, superficial and deep endometriosis lesions reported in the literature. It reiterates the low sensitivity of pelvic ultrasound for the diagnosis of superficial endometriosis, its high sensitivity and specificity for ovarian or rectal locations, its lower sensitivity and specificity for sigmoid locations, and its low sensitivity but high specificity for lateral locations. The benefits of using standardized reports are highlighted, as they allow for a more consistent description of abnormalities, a more comprehensive description, and contribute to improved multidisciplinary communication and optimized patient care. The Haute Autorité de santé also emphasizes the importance of practitioner training and the need for a common terminology to improve the reproducibility and quality of diagnoses.
