Invasive lobular carcinoma: Clinicopathologic and biological insights

Le carcinome lobulaire infiltrant (CLI), type histologique le plus fréquent de cancer du sein après le carcinome infiltrant de type non spécifique, constitue une entité clinicopathologique particulière. Il se caractérise par une inactivation du gène de la E-cadhérine ( CDH1 ), un aspect non cohésif des cellules tumorales, des lésions mal délimitées, fréquemment multifocales et bilatérales, et un tropisme métastatique particulier (péritoine, méninges, ovaires, tractus gastro-intestinal). Des avancées récentes ont identifié des spécificités biologiques propres aux cellules tumorales lobulaires, soutenant le développement de thérapies ciblées anti-CLI. Dans ce contexte, l’amélioration de la reproductibilité diagnostique, encore insuffisante, représente un enjeu important pour le pathologiste. Des recommandations européennes ont ainsi proposé en 2024 de standardiser les pratiques diagnostiques, notamment en codifiant l’utilisation de l’immunohistochimie. Cette revue synthétise l’état de l’art diagnostique, biologique et thérapeutique du CLI, au cœur d’un renouveau d’intérêt en sénologie.

Invasive lobular carcinoma (ILC), the second most common histologic subtype of breast cancer, is a clinicopathologic entity distinct from invasive carcinoma of no special type. It is defined by loss of E-cadherin (inactivation of CDH1 gene), which underlies the characteristic discohesive tumor cells, and is associated with ill-defined growth pattern, frequent multifocality and bilaterality, and a distinctive pattern of metastases (peritoneum, meninges, ovaries, gastrointestinal tract). Recent work has delineated several lobular-specific biological features, supporting the development of ILC-directed targeted therapies. In this context, improving diagnostic reproducibility (still suboptimal) remains a priority for pathologists. In 2024, the European Lobular Breast Cancer Consortium called for standardization of diagnostic practice, including codified use of immunohistochemistry. This review synthesizes the current diagnostic, biological, and therapeutic landscape of ILC amid renewed interest within the medical and scientific community.
