Combined epidural and intracerebral hematomas after evacuation of bilateral chronic subdural hematoma - 12/12/08

Abstract |
Bleeding after surgery for chronic subdural hematoma far from the operative site is a rare phenomenon with possibly serious consequences. We report a case of combined epidural and intracerebral hemorrhage immediately after evacuation of bilateral chronic subdural hematoma. The epidural hematoma was evacuated by emergency craniotomy, but the deep parenchymal hematoma was treated conservatively. The patient recovered progressively with a good outcome. Approximately 30cases of chronic subdural hematomas complicated by intracerebral hematoma were previously reported, but only seven cases of epidural hematoma. These complications could be avoided if slow, gradual decompression is used during surgery. Clinicians should suspect its occurrence without delay when a postoperative neurological deterioration is demonstrated. Possible mechanisms are discussed.

La survenue d’un saignement après chirurgie d’un hématome sous-dural chronique à distance du site opératoire est un phénomène rare et potentiellement grave. Nous rapportons le cas original de la combinaison d’un hématome extradural et intracérébral immédiatement après évacuation d’un hématome sous-dural chronique bilatéral. L’hématome épidural a été évacué en urgence, l’hématome intracérébral traité de façon conservative. Le patient a eu une récupération lente, suivie d’un bon résultat. Environ 30cas d’hématomes sous-duraux chroniques, compliqués en postopératoire d’un saignement intracérébral ont été déjà rapportés, et sept autres se compliquant d’hématome épidural. Ces complications sont prévenues par une évacuation lente de l’hématome sous-dural. En pratique neurochirurgicale, les praticiens doivent suspecter cette rare complication devant toute aggravation neurologique. Les différents mécanismes seront discutés concernant l’apparition de cette complication.
