Résumé |
Objectif de l’étude |
Évaluer l’intérêt de la radiothérapie dans la prévention de l’ossification hétérotopique.
Patients et méthodes |
Vingt-deux patients âgés de 20 à 76ans ont reçu une radiothérapie pour la prévention des ossifications hétérotopiques. Tous les patients ont reçu une radiothérapie postopératoire de 7Gy par photons de 18MV. Le délai entre la chirurgie et la radiothérapie a été de 24heures chez 20 patients, 48heures chez trois patients, 72heures chez deux patients et 96heures chez un patient. Les critères utilisés pour le suivi des patients étaient le stade radiologique (score de Brooker) et l’état fonctionnel de l’articulation avant et après la chirurgie. Parmi les 14 patients suivis, sept ont reçu une radiothérapie primaire et sept une radiothérapie secondaire.
Résultats |
La prophylaxie de l’ossification hétérotopique par radiothérapie a été efficace sur le plan radiologique chez 71,5 % des patients : 86 % dans le cas de prophylaxie primaire et 57 % en cas de prophylaxie secondaire. Aucun échec sur le plan fonctionnel n’a été noté. Aucun patient n’a eu des effets secondaires.
Conclusion |
La radiothérapie est efficace comme prophylaxie de l’ossification hétérotopique. Un plus grand nombre de patients est nécessaire pour pouvoir comparer nos résultats à ceux de la littérature.

Purpose |
To evaluate the use of radiation therapy for the prevention of heterotopic ossification.
Patients and methods |
Twenty-two patients, 20 to 76 years old were irradiated for the prevention of heterotopic ossification. All patients received a postoperative irradiation consisting of 700cGy delivered by 18MV photons using a linear accelerator. The delay between surgery and irradiation was 24hours for 20 patients, 48hours for three patients, 72hours for two patients and 96hours for one patient. The follow-up of all patients was based on radiologic findings (Brooker score) as well as the clinical status of the articulation before and after surgery. Of the 14 cases for which follow-up data was available, seven received primary prophylaxis and seven received secondary prophylaxis.
Results |
Heterotopic ossification prophylaxis using radiation therapy was radiologically successful in 71.5% of patients: 85.7% success for patients receiving primary prophylaxis and 57.1% success for patients receiving secondary prophylaxis. No functional failure was reported. No patient developed radiation related side effects.
Conclusion |
Radiation therapy is proven to be effective in preventing heterotopic ossification. A larger study group is needed in order to compare our results with those seen in the literature.
