Évaluer le caractère minimal invasif et la courbe d’apprentissage du Prolift™.
Patientes et méthodes |
Étude prospective, toutes les patientes étaient opérées par le même chirurgien. Le caractère minimal invasif a été étudié en analysant les complications per- et postopératoires immédiates. La courbe d’apprentissage était étudiée en analysant le temps opératoire.
Résultats |
Entre janvier et décembre 2007, 47 patientes ont été inclues. Le recul moyen de surveillance était de 11,8 mois. Deux plaies de vessie et deux hémorragies peropératoires supérieures à 500ml ont été signalées. Une érosion vaginale et une récidive de l’étage moyen ont été rapportées. Le temps opératoire pour un Prolift™ total était 62±18minutes. Le temps antérieur était de 40±8minutes, ce temps est resté stable après la dixième intervention. Le temps postérieur a duré en moyenne 24±13minutes, il est resté stable après la 18e intervention.
Discussion |
L’apprentissage du temps postérieur est plus long du fait des passages des allènes dans les fosses ischirectale. La technique semble être minimale invasive, sa courbe d’apprentissage est faible.
Conclusion |
Un suivi à plus long terme est indispensable pour juger de son efficacité dans le temps.

Aims |
Evaluation of the mini invasiveness and the learning curve of the Prolift™ technique.
Materials and methods |
Prospective study. All patients were operated on by the same surgeon. The mini-invasiveness of the procedure was estimated through the evaluation of the intraoperative and immediate postoperative complications. The learning curve was evaluated through the analysis of the operative time.
Results |
Between January and December 2007. Forty-seven patients were included in the study. Mean follow-up was: 11,8 months. Two cases of bladder injury and two cases of intraoperative bleeding (>500ml) were reported. One case of vaginal erosion and one case of recurrence of the prolapse occurred during the follow-up. The mean operative time was 62±18min. The mean operative time of the posterior step of the Prolift™ was 24±min and remained stable after the 18th procedure.
Discussion |
The learning cure of the posterior of the procedure is longer because of the passage of the needles through the ischiorectal foramens. The technique is mini-invasive considered the low rate of intra and immediate postoperative complication and the learning curve short.
Conclusions |
Longer follow-up is needed to evaluate the efficacy of the procedure in the long term.
