Résumé |
Le rivaroxaban (Xarelto®) est une molécule inhibant de manière directe le facteur Xa de la coagulation, hautement sélective et administrée par voie orale.
Les principales caractéristiques pharmacocinétiques du rivaroxaban sont une biodisponibilité d’environ 80 -100 %, une concentration maximale obtenue en 2 à 4 heures, une demi-vie d’élimination terminale de 7 à 11 heures, une élimination rénale directe sous forme active pour 1/3 et métabolisation hépatique par la voie des cytochromes P450 (3A4) pour 2/3.
Les principales sources de variabilité sont la fonction rénale, la fonction hépatique et les inducteurs et inhibiteurs puissants du CYP450 3A4. Les études de phase II ont montré que ce produit peut être administré par voie orale en 1 ou 2 prises quotidiennes, sans surveillance biologique ni ajustement posologique, qu’il est contre-indiqué en cas d’insuffisance hépatique sévère et non recommandé pour les insuffisances rénales sévères.
Sur le plan pharmacodynamique, il s’agit d’un inhibiteur direct et sélectif du facteur Xa, sans activité anti facteur IIa et sans interaction avec les plaquettes sanguines.
Quatre phases II avec 2787 patients ont été réalisées dans la prévention de la maladie thromboembolique veineuse (MTEV) après chirurgie orthopédique majeure montrant que la posologie de 10 mg × 1 fois par jour était la dose optimale à tester en phase III. Deux études de phase II pour le traitement de la MTEV ont été réalisées avec 1446 patients. montrant que la posologie optimale à tester en phase III était de 15mg×2/jour pendant 3 semaines puis 20mg×1/jour au delà. Aucun signal de toxicité hépatique grave n’a été noté dans ces études de phase II.

Rivaroxaban (Xarelto®) is a new oral, direct and selective inhibitor of the factor Xa of the coagulation cascade.
The main pharmacokinetic characteristics of rivaroxaban are a bioavailability of approximately 80-100%, a maximum concentration obtained in 2 to 4 hours, a terminal half-life of elimination of 7 to 11 hours, a renal elimination for 1/3 for the active hepatic metobolism from the cytochrome P450 (3A4) for the other 2/3. The main sources of variability are the renal and the liver function and potential interactions with some strong inhibitors or inducers of the CYP450 3A4. Phase II clinical studies have shown that this compound can be orally administrated, once or twice daily, without any biological monitoring and without any need for dose adjustment. There is a contra-indication in case of severe liver insufficiency and not recommended in case of severe renal impairment.
Pharmacodynamically, Rivaroxaban is a direct and selective factor Xa inhibitor without any effect on the factor IIa and without any interaction on platelets.
Four phases II with 2787 patients were carried out to for venous thromboembolic (VTE) prophylaxis after major orthopaedic surgery, showing that 10 mg once daily could be the optimal dose regimen to assess in phase III.
Two phases II with 1446 patients were carried out for the treatment of VTE showing that 15 mg twice daily for 3 weeks and then 20 mg once daily could be the optimal dose regimen to evaluate in the following phases 3. No strong signal for a potential liver toxicity was shown during these 6 phases II.
