Initialement, la chimiothérapie première avait pour objectifs d'allonger la survie par son action précoce sur les micro-métastases, d'évaluer in vivo la chimiosensibilité et d'adapter le traitement postopératoire, d'augmenter le nombre de traitements conservateurs. Huit essais randomisés comparant chimiothérapie première et adjuvante ont été publiés. Le consensus général concernant ces essais est que la chimiothérapie première n'allonge pas la survie, en comparaison d'une chimiothérapie adjuvante. Il y a une corrélation entre le taux de réponse clinique et surtout histologique et une meilleure probabilité de survie, ce qui permet de disposer donc d'un facteur pronostique de première importance. Elle a pour principal avantage de permettre l'étude des facteurs de prédiction, en particulier biologiques, de réponse à la chimiothérapie, mais l'adaptation des thérapeutiques adjuvantes à cette réponse première reste à évaluer. Enfin, elle résulte en une augmentation modeste du nombre de traitement conservateur, mais elle expose à un risque accru de récidive locale à long terme. Proposer un traitement conservateur après chimiothérapie première chez une patiente atteinte d'un cancer initialement justiciable d'une mastectomie nécessite de l'en informer sur ce risque accru.

Abstract |
Initially, preoperative chemotherapy had three aims, to improve the overall survival by an early exposure to systemic therapy, to evaluate in vivo the efficacy of chemotherapy and to monitor post-operative treatment, and to increase the rate of breast-conserving treatments. Eight randomised trials have been published. The general consensus about the results of these trials is that neoadjuvant chemotherapy did not improve overall survival, in comparison with an adjuvant treatment. Clinical and especially pathological complete response rates were correlated with the outcome of the patients and could be considered as an independent prognostic factor. Primary chemotherapy is a powerful tool to study the predictive factors, especially biological factors, of chemosensitivity. However, the usefulness of this approach to adjust post-operative treatments remains to be demonstrated. Finally, primary chemotherapy slightly increased the rate of breast conserving treatments, but is accompanied by a small increase in the risk of local recurrence. To propose a conservative treatment to a woman initially not suitable for a lumpectomy needs clear information about this enhanced risk.
