Transfert embryonnaire en cas de col sténosé et/ou infranchissable - 30/10/09

Résumé |
Une sténose cervicale peut s’observer après une infection, une conisation ou une amputation du col. Elle peut s’accompagner d’une stérilité cervicale. En fécondation in vitro (FIV), les transferts d’embryons sur col sténosé sont difficiles et s’accompagnent de taux de grossesse diminués justifiant une prise en charge. Les efforts thérapeutiques tentent de restaurer une perméabilité satisfaisante. Des dilatations cervicales itératives peuvent être une solution. En cas d’impossibilité à obtenir une perméabilité adéquate, un néo-orifice et/ou un néo-canal cervical doivent être créés chirurgicalement. La pérennité du résultat est assurée par des dilatations post chirurgicales ou par un cathéter intracervical laissé en place jusqu’à cicatrisation. En cas d’échec de ces traitements du col, des alternatives thérapeutiques peuvent être proposées. L’insémination intrapéritonéale dans le cul de sac de Douglas est intéressante à condition que les trompes soient perméables et que la qualité du sperme soit suffisante. Dans le cas contraire, le transfert d’embryons transmyométrial et le transfert intratubaire de gamètes ou de zygotes représentent une ultime option thérapeutique.

A cervical stenosis can follow an infection, a conisation or a trachelectomy and lead to a cervical infertility. In in vitro fertilization (IVF), embryo transfers are difficult in case of a cervical stenosis and specific care is required. Treatments try to restore an adequate cervical patency. Repeated cervical dilatations can represent a solution. In case of inefficiency, a new cervicoisthmic opening has to be created with surgery. Perpetuity of the result is obtained thanks to post surgical dilatations or thanks to implementation of an intracervical catheter till complete cicatrization. In case of failure of canalization of the stenosed isthmic os, alternative treatments can be proposed. Intraperitoneal insemination in the poutch of Douglas is interesting when tubes are patent and when semen quality is sufficient. On the contrary, transmyometrial embryo transfer and gametes or zygotes intrafallopian transfer represent an ultimate therapeutic option.
