Résumé |
Le développement de nouveaux médicaments antiarythmiques concerne surtout le traitement de la fibrillation auriculaire à cause de sa prévalence et des conséquences thromboemboliques majeures qu’elle engendre. La proarythmogénicité induite par un blocage « pur » des courants INa et IK, même s’ils sont porteurs d’une efficacité antiarythmisante certaine, ont fait se détourner industriels et cliniciens de ces modes d’action pharmacologiques au profit d’un blocage préférentiel de courants « auriculaires » comme IKur ou IKAch. L’efficacité démontrée de l’amiodarone dans la fibrillation auriculaire a fait aussi que toute une série de bloqueurs « multicanaux » ciblent à la fois ces nouveaux courants en gardant toutefois certaines propriétés pharmacologiques de l’amiodarone, ce qui rend caduque la classification de Vaughan-Williams pour ces nouveaux traitements.

The development of new antiarrhythmic drugs is mainly aimed to treat atrial fibrillation, because of its prevalence and major consequences in terms cerebral vascular thrombosis. Specific blockade of INa et IK, even if efficacious, have previously shown to be proarrhythmogenic, with a global impairment of the cardiac patient’s outcome. This lead to the development of new drugs, selectively targeting atrial currents such as IKur ou IKAch. The efficacy of amiodarone in treatment of atrial fibrillation has also yielded a whole array of new antiarrhythmic drugs targeting both these atrial currents but also sharing amiodarone pharmacodynamics properties. This renders the Vaughan-Williams classification ill-adapted for such drugs.
