Syndrome de Vogt-Koyanagi-Harada au cours d’une hépatite C chronique sous interféron alpha et ribavirine - 17/03/10

Les effets immunologiques complexes de la bithérapie associant interféron et ribavirine au cours du traitement de l’hépatite C peuvent aggraver ou déclencher une réaction auto-immunitaire. Nous rapportons un cas rare de maladie de Vogt-Koyanagi-Harada survenue chez un patient suivi pour une hépatite C chronique après institution d’un traitement associant de l’interféron-alpha2a et de la ribavirine. Le rôle potentiel de l’interféron-alpha et/ou de la ribavirine est discuté, en insistant sur l’association entre le traitement de l’hépatite C par interféron-alpha/ribavirine et le développement de la maladie de Vogt-Koyanagi-Harada. Une telle complication ophtalmologique grave exige un suivi ophtalmologique étroit de tous les patients ayant une hépatite C traitée par interféron-alpha.

The complex immunological effects of interferon and ribavirin therapy in hepatitis C virus may also exacerbate or trigger the development of autoimmunity. A rare case of Vogt-Koyanagi-Harada disease associated with ribavirin and interferon-alpha treatment for chronic hepatitis C infection is presented. The potential role of interferon and/or ribavirin therapy is discussed. Physicians should be aware of the association between interferon-alpha 2a and ribavirin use for hepatitis C infection and the development of Harada disease. This severe ophthalmological complication requires close follow-up of hepatitis C virus-infected patients on interferon-alpha treatment.
