Ischémie digitale par embolie à partir d’un abord d’hémodialyse thrombosé : une complication rare à ne pas méconnaître - 13/04/10

Un abord d’hémodialyse thrombosé est habituellement laissé en place en raison de sa bonne tolérance clinique. Cependant, il peut être à l’origine d’embols distaux et entraîner un tableau d’ischémie en aval de l’abord. Nous rapportons le cas d’un patient de 43 ans, transplanté rénal, qui consultait pour une ischémie digitale de la main droite. Ce patient était porteur d’une fistule artérioveineuse au poignet droit, thrombosée depuis cinq ans. L’ischémie était secondaire à des embols distaux libérés à partir du thrombus constitué au site de l’anastomose artérioveineuse. Un traitement par antiagrégant plaquettaire et anticoagulant a été mis en route permettant la régression des signes cliniques et l’absence de récidive, puis la fistule a été ligaturée chirurgicalement. Par la suite, seul le traitement antiagrégant a été maintenu. Avec un recul de six mois, le patient est resté asymptomatique. Cette observation souligne l’importance de la surveillance clinique des abords thrombosés qui pourrait conduire à la ligature chirurgicale préventive en cas de risque embolique élevé.

Surgical removal of a hemodialysis access after thrombosis is generally not performed as it remains clinically well tolerated. However, it may be the source of distal embolization. We report the case of a 43-year-old patient, kidney recipient, who presented with digital ischemia of the right hand. He had a forearm arteriovenous fistula at the right wrist which thrombosed 5 years ago. Digital ischemia was due to thrombus formation at the anastomotic site and migration into the downstream arterial bed. Heparine was initiated together with antiplatelet treatment. The ischemia resolved after a few days, no recidive was observed. Surgical ligation of the arteriovenous fistula was rapidly performed and antiplatelet treatment was maintained after surgery. After a follow-up of 6 months, the patient remained asymptomatic without new embolization. This observation underlines the necessity of clinical monitoring after access thrombosis and preventive surgical ligation might be discussed when the risk of distal embolization is high.
