Péritonite encapsulante révélatrice d'un mélanome primitif du grêle : à propos d'une observation - 01/01/04

La péritonite encapsulante est une péritonite chronique aboutissant à une membrane fibreuse épaisse blanche nacrée. C'est une affection rare dont la physiopathologie reste mal expliquée et le diagnostic souvent porté en peropératoire. Nous rapportons l'observation d'un patient âgé de 36 ans hospitalisé pour une masse abdominale dont les différentes explorations radiographiques ont conclu à une tumeur digestive avec encapsulation du grêle. Le diagnostic a été opératoire par la découverte d'une péritonite encapsulante englobant le jéjunum et l'iléon en totalité et dont l'origine est un mélanome malin primitif du grêle, tumeur digestive rare dont la localisation grêlique est exceptionnelle.

Abstract |
Encapsulating peritonitis is a chronic peritonitis leading to the constitution of the thick white nacred fibrosis membrane. It is a rare affection. Its physiopathology is poorly elucidated. Diagnosis is usually at operation. We report an unpublished case of a 36-year-old patient, admitted for an abdominal mass. The different investigations demonstrated small bowel tumor with entrapped ileal and jejunal loops within a capsule. Diagnosis was made at the time of operation by discovering an encapsulating peritonitis caused by a primary malignant melanoma of the small bowel, which is an exceptional affection.
