Résumé |
Dans le cadre de la prise en charge des patients atteints de carcinome thyroïdien différencié (CDT), il est toujours important de pouvoir localiser avec précision les localisations métastatiques. Malheureusement, la scintigraphie planaire seule fait parfois défaut sur ce plan, vraisemblablement par manque de repères anatomiques. Dans ce contexte, l’imagerie en coupes de type TEMP-TDM apparaît comme une technique privilégiée permettant de pallier cette insuffisance. Nous rapportons le cas d’un patient opéré pour carcinome vésiculaire de la thyroïde métastatique, chez qui la scintigraphie planaire a permis le diagnostic de foyers iodofixants multiples avec localisation anatomiquement non précise au niveau abdominal et que l’imagerie TEMP-TDM a permis de situer et de rattacher à une localisation hépatique.

It is very important in the management of patients with differentiated thyroid cancer (CDT) to precisely localize the foci of I-131 uptake, but it is difficult with planar scintigraphy only because of a lack of anatomic landmarks. In this context hybrid-imaging appears to be a preferred technique to overcome this deficiency. We report a case of metastatic follicular thyroid carcinoma (CDT) in which planar scintigraphy diagnosed multiples uptake abnormalities. The foci of I-131 pinpointed in abdominal cavity could not be identified anatomically on whole body scintigraphy. SPECT-CT imaging helped to locate these hot spots and assign them to hepatic localization.
