Résumé |
Introduction – Le but de ce travail est de vérifier s'il est possible d'accroı̂tre l'efficacité du réentraı̂nement sur la réhabilitation des bronchiteux chroniques obstructifs, en y adjoignant une supplémentation orale en acides aminés ramifiés.
Synthèse des faits – Trente patients bronchitiques chroniques obstructifs réentraı̂nés ont été supplémentés en acides aminés ramifiés et appariés à 30 témoins (patients identiques suivant le même réentraı̂nement) non supplémentés. Les deux groupes ont amélioré leur puissance maximale atteinte et leur VO2sl. Seuls les patients supplémentés ont amélioré leur PaO2 au repos de façon hautement significative, et développé une hypocapnie traduisant une alcalose ventilatoire. La discussion du mécanisme responsable amène à privilégier l'hypothèse d'une action directe des acides aminés ramifiés sur les centres respiratoires.
Conclusion – une administration systématique d'acides aminés ramifiés pourrait accroı̂tre l'efficacité de l'entraı̂nement sur l'amélioration de la fonction respiratoire chez des bronchitiques chroniques obstructifs.

Introduction – The aim of this work was to establish if it is possible to improve the efficacy of retraining on rehabilitation of patients suffering from chronic obstructive bronchitis, by oral supply in branched-chain aminoacids.
Synthesis of facts – Thirty retrained patients with chronic obstructive bronchitis were supplied in branched-chain aminoacids and matched with 30 retrained witnesses (identical patients) without additional supply in branched-chain aminoacids. The two groups improved their reached maximal power and their VO2SL. Only the supplied patients improved their PaO2 at rest highly significantly, and developed hypocapnia expressing a gaseous alkalosis. The discussion of the responsible mechanism leads to favour the hypothesis of a direct action of branched-chain aminoacids on the respiratory centres. This observation could have practical outcomes.
Conclusion – it should be useful to systematically supplement the patients with branched-chain aminoacids during their retraining in order to obtain a more effective improvement of their respiratory function.
