Traitement chirurgical de l'inertie colique - 01/01/04

Le traitement chirurgical de l'inertie colique concerne moins de 10 % des patients souffrant de constipation sévère résistant au traitement médical. Un bilan préopératoire colorectal exhaustif morphologique et manométrique permet de mieux sélectionner les patients. La colectomie subtotale suivie d'une anastomose iléorectale est l'intervention habituellement réalisée ; la laparoscopie a probablement une place dans cette indication. Le traitement chirurgical permet une amélioration significative de la symptomatologie chez près de 2/3 des patients. Toutefois il expose à un risque d'occlusion intestinale chez 1/4 des patients, de douleurs abdominales chez plus de la moitié d'entre eux, voir d'échec chez près d'un malade sur dix.

Less than 10% of patients with slow-transit constipation require surgical management after failure of medical treatment. Preoperative clinical, psychological and colorectal routine investigations (ie colonic transit test, anorectal manometry and defecography) are mandatory in order to highly select the patients. To day, the surgical management of slow-transit constipation consists of subtotal colectomy with ileorectal anastomosis, eventually by laparoscopic approach. Although, surgical management improves slow-transit constipation in two thirds of the patients, small bowel obstruction, abdominal pain and constipation recurrence can occur in 25%, 50%, and 10% of the patients respectively.
