Interruption médicale de grossesse et placenta praevia, intérêt d’un fœticide précoce ? - 22/03/11

Objectifs |
Étudier la morbidité maternelle d’un accouchement par voie basse en cas d’interruption médicale de grossesse (IMG) associée à un placenta praevia et étudier l’intérêt d’un fœticide précoce (réalisé quelques jours avant le déclenchement).
Patientes et méthodes |
Il s’agit d’une étude rétrospective cas–témoins avec deux groupes de patientes appariées : le groupe des cas (IMG associée à un placenta praevia) et le groupe des témoins (IMG et placenta normalement inséré). La morbidité maternelle définie par l’hémorragie pendant l’IMG, l’hémorragie de la délivrance, la transfusion sanguine et la perte moyenne d’hémoglobine a été comparée entre les deux groupes et au sein du groupe des cas, entre les patientes ayant bénéficié d’un fœticide précoce et celles n’ayant pas eu de fœticide précoce.
Résultats |
De 1996 à 2009, nous avons identifié 15 cas d’IMG associées à un placenta praevia (sept placentas recouvrant et huit placentas bas insérés) qui ont été appariés à 29 cas d’IMG avec un placenta normalement inséré. Parmi le groupe des cas, huit patientes ont bénéficié d’un fœticide précoce, six patientes n’ont pas eu de fœticide et une patiente a eu un fœticide le jour du déclenchement. La morbidité maternelle était accrue de manière significative dans le groupe des placentas praevia par rapport au groupe des placentas normalement insérés (quatre hémorragies pendant l’IMG, deux hémorragies de la délivrance, quatre patientes transfusées et perte moyenne d’hémoglobine de 1,5g/dL dans le groupe des cas versus aucun cas d’hémorragie ni de transfusion et perte moyenne d’hémoglobine de 0,5g/dL dans le groupe témoin ; p<0,05). Dans le groupe fœticide précoce, la morbidité maternelle avait tendance à diminuer sans que ce résultat soit significatif.
Conclusion |
L’accouchement par voie basse en cas d’IMG associée à un placenta praevia même recouvrant semble une option raisonnable ; la réalisation d’un fœticide précoce reste à être évaluée.

Objectives |
Study of maternal morbidity when termination of pregnancy (TOP) is associated with placenta previa and study of the interest of performing feticide before labor induction.
Patients and methods |
This is a retrospective case control study with two groups of matched women: the case group (women with placenta previa) and the control group (women without placenta previa). Maternal morbidity have been studied and compared between the two groups and in the case group, between women who had feticide between labor induction and women who did not have feticide.
Results |
Between 1996 and 2009, we identified 15 cases of TOP with placenta previa (seven complete placentas previa and eight low lying placenta including partial and marginal placenta previa) who have been matched with 29 women. In the case group, eight women had feticide before labor induction, six women had no feticide and one woman had feticide the same day of labor induction. Maternal morbidity was increased in this group but without major complications (four hemorrhages during labor, two postpartum hemorrhages, four transfused patients and mean difference of hemoglobin level was 1,5g/dl in the case group versus no hemorrhage during labor, no postpartum hemorrhage, no transfusion and mean difference of hemoglobin level was 0,5g/dl in the control group; P<0,005). Performing feticide before labor induction allowed a non-significant reduction of mean loss of hemoglobin and of the number of women who needed transfusions.
Conclusion |
A vaginal delivery in cases of TOP with placenta previa and even totally recovering seems a reasonable solution; performing feticide before labor induction could decrease loss of blood but further studies are needed.
