Le PRF (Platelet Rich Fibrin) appartient à une nouvelle génération de concentrés plaquettaires recherchant des modes de production simplifiés et sans manipulation biochimique du sang (héparine, EDTA, thrombine bovine, Chlorure de calcium...). Dans ce quatrième article, nous avons cherché à corréler la biologie précédemment évaluée du PRF avec de premiers résultats cliniques établis, afin de déterminer les champs d'applications potentielles de ce biomatériau. Notre raisonnement s'articule autour de quatre événements fondamentaux de la cicatrisation : l'angiogenèse, le contrôle immunitaire, la captation des cellules souches circulantes et l'épithélialisation de couverture de la plaie. En effet, toutes les applications cliniques connues du PRF mettent en évidence une cicatrisation accélérée des tissus grâce au développement d'une néovascularisation efficace, d'une fermeture accélérée des plaies avec remodelage rapide du tissu cicatriciel et d'une absence quasi totale de suites infectieuses. Ces premiers travaux permettent donc d'envisager de nombreuses applications à venir pour le PRF, tant en chirurgie plastique qu'en chirurgie osseuse, à condition d'être capable d'en évaluer de manière impartiale et rigoureuse les effets réels.

The PRF (Platelet Rich Fibrin) belongs to a new generation of platelet concentrates, with simplified modes of production and without biochemical handling of blood (heparin, EDTA, bovine thrombin, Calcium chloride...). In this fourth article, we sought to correlate the previously evaluated biology of the PRF with first established clinical results, in order to determine the potential fields of application of this biomaterial. Our reasoning is structured around four fundamental events of the cicatrisation: angiogenesis, immune control, circulating stem cells trapping and wound covering epithelialisation. Indeed, all the known clinical applications of the PRF highlight an accelerated tissue cicatrisation because of the development of an effective neovascularization, an accelerated wound closing with fast cicatricial tissue remodelling and the quasi-total absence of infectious events. This first work thus makes it possible to plan many future applications for the PRF, as well in plastic surgery as in bone surgery, provided that its real effects are evaluated in an impartial and rigorous way.
