Évaluation de l’analyse peropératoire des ganglions « sentinelle » par biologie moléculaire (technique Gene Search BLN Assay) dans le cancer du sein : à propos de 126 patientes - 27/05/11

La technique de biologie moléculaire RT-PCR en temps réel, Gene Search BLN Assay (BLN), permet de détecter en peropératoire les métastases des ganglions sentinelles (GS) dans les carcinomes mammaires. Notre objectif a été de la comparer à l’analyse conventionnelle définitive et d’évaluer sa faisabilité en routine.
Matériel et méthodes |
Chaque GS était sectionné en plusieurs tranches. Une tranche sur deux était analysée en peropératoire par BLN, l’autre moitié par histologie conventionnelle définitive.
Résultats |
Deux cent trente-quatre GS ont été analysés chez 124 patientes. Trente-cinq GS étaient envahis chez 29 patientes (23,4 %). Le diagnostic a été posé 28 fois en peropératoire. Deux cas de discordance entre BLN et l’histologie définitive ont été observés, probablement relatifs à l’échantillonnage. La sensibilité de BLN est de 96,4 %, la spécificité de 99 %, la valeur prédictive positive de 96,4 %, la valeur prédictive négative de 99,0 % et la concordance de 98,4 %. La durée des interventions est augmentée et nécessite une adaptation au bloc opératoire.
Conclusion |
La technique BLN offre un intérêt majeur pour la patiente, le chirurgien et le pathologiste en raison de la meilleure qualité du résultat rendu en peropératoire comparée à l’examen extemporané conventionnel.

Introduction |
Intraoperative molecular assay Gene Search BLN Assay (BLN) detects sentinel lymph node (SLN) metastasis in breast cancer. Our objective was to compare BLN to the definitive conventional histologic methods and to experiment the management of BLN in routine.
Materiel and methods |
Each SLN was cut into alternate slabs. Half slabs were analysed with the intraoperative BLN molecular method, and the other slabs with the definitive histologic method.
Results |
Two hundred and thirty four SLN have been analysed (124 patients). Thirty-five SLN had metastasis for 29 patients (23.4%). BLN correctly identified 28 patients. Two cases of discordance between BLN and standard method were found, probably explained by a sample bias. The sensibility of BLN is 96.4%, the sensitivity is 99%, the predictive positive value is 96.4%, the predictive negative value is 99% and the concordance is 98.4%. The surgery time increases and there is a need to adapt the theatre organization accordingly.
Conclusion |
The Gene Search BLN Assay gives a great interest for the patient, the surgeon and the pathologist because it increases the quality of the intraoperative analysis by comparison with the intraoperative conventional histology.
