Obstacle atypique lors d’un changement de cathéter d’hémodialyse - 02/06/11

Une patiente de 60 ans est suivie dans notre service pour insuffisance rénale chronique d’origine vasculaire. La mise initiale en dialyse péritonéale s’est compliquée précocement d’une infection à Candida albicans et cette technique a dû être abandonnée. Nous lui avons donc posé un cathéter jugulaire interne droit tunnelisé (Medcomp 32cm), devant le refus de la création d’une fistule artérioveineuse par la patiente. Le cuff du cathéter devenant de plus en plus mobile, il a été décidé de le remplacer par un autre cathéter tunnelisé (Quinton 40cm). La présence d’un volumineux manchon fibrocalcaire a compliqué la manœuvre nécessitant calme et agilité. La taille de cette calcification reste rare et reflète le risque majeur des calcifications chez les insuffisants rénaux chroniques.

We followed a 60-year-old woman in our department for a vascular chronic kidney disease. The peritoneal dialysis was stopped because of a Candida albicans catheter infection. As the patient refused the arterio-venous fistula creation, we have placed a central venous catheter (Medcomp 32cm). Because the instability of the cuff, we decided to remove and change it by another one (Quinton 40cm). The discovery of an important cylindrical calcification has complicated the manipulation requiring tranquility and agility. The finding of a massive calcification is uncommon and confirms the major risk of calcification in chronic kidney disease patients.
