Introduction |
Through clinical pharmacy, the pharmacist can contribute to the continuous improvement of care for hospitalized patients. The objective of the present study was to conduct a descriptive analysis of the pharmaceutical interventions (PI) on medical prescriptions and assess their relevance.
Methodology |
A prospective and noncomparative study was conducted over a period of 6months in the oncology and haematology ward in one French teaching hospital. A pharmacy student integrated this ward, part-time, during the medical rounds and performed the analysis of all the prescriptions issued and did a PI when appropriate. The methodology developed and validated by the French Society of Clinical Pharmacy (SFPC) was used. Moreover, the clinical impact of these addresses was evaluated.
Results |
During the study period, 22.4% of the patients were identified with concern to 188 PI. The acceptance rate of PI by physicians on drug prescriptions was comparable to previously published data; most of these have a clinical impact considered to be at least significant. With regards to the clinical significance of PI, medical or pharmaceutical points of view exhibited no statistical differences (Student t test).
Discussion/conclusion |
The results reinforce the idea that oral communication and a regular presence in the unit encourage the integration of the pharmacists in health care teams. The present study has enabled the researchers to target treatment at risk and show that pharmacotherapeutic problems may have significant clinical impact for the patient.

Introduction |
Tout pharmacien sensibilisé à la pharmacie clinique peut contribuer à l’amélioration continue de la prise en charge des patients hospitalisés. L’objectif principal de ce travail était de réaliser une analyse descriptive des interventions pharmaceutiques (IP) sur les prescriptions et d’en évaluer la pertinence.
Méthodologie |
Une étude prospective non comparative a été menée sur une période de six mois dans le service d’hospitalisation continue de cancérologie de notre CHRU. Une interne a suivi la visite des médecins, procédé à l’analyse de l’ensemble des prescriptions et a émis des IP le cas échéant. L’outil de codification et d’évaluation mis au point et validé par la SFPC a été utilisé. De plus, l’impact clinique potentiel des IP a été estimé.
Résultats |
Parmi les IP concernant 22,4 % des patients hospitalisés, 188 ont été émises. Le taux d’acceptation médicale des IP a été comparable à celui de travaux analogues, dont une majorité a un impact clinique estimé comme étant au moins significatif. Il n’y a pas de différence significative entre la cotation de l’impact clinique du médecin et celle du pharmacien (test de Student).
Discussion/conclusion |
Ces résultats montrent qu’une communication orale et qu’une présence régulière dans le service améliorent l’efficacité des actions pharmaceutiques. Ce travail nous a également permis de cibler les traitements à risque ainsi que les problèmes pharmacothérapeutiques susceptibles d’avoir un impact clinique important.
