Comme chez l'adulte, la surveillance (ou monitorage) des traumatismes crâniens graves de l'enfant a trois objectifs : prévenir ou limiter la survenue de lésions secondaires, guider au mieux la thérapeutique et apporter des arguments pronostiques. Outre les examens cliniques répétés et l'imagerie cérébrale, la surveillance continue non spécifique des paramètres suivants est indispensable : PA, température, PaO2 (SpO2), PaCO2 (FETCO2), hémoglobine, hématocrite. Le monitorage spécifique du cerveau associe la surveillance de la perfusion cérébrale (PIC, doppler transcrânien) de l'activité électrique du cerveau (EEG) et dans certains cas de l'oxygénation cérébrale (SvjO2). L'interprétation des données du monitorage doit toujours être prudente. Un enfant traumatisé crânien grave en réanimation justifie une présence médicale compétente de tous les instants.

Abstract |
As in the case of adults, there are three main goals in the monitoring of severe head trauma in children: to prevent or minimize the apparition of secondary lesions, to optimize treatment, to help make precise prognosis.The basic monitoring is composed of repeated clinical examinations, brain radiological imaging and control of vital parameters (blood pressure, temperature, PaO2 (SpO2), PaCO2 (FETCO2), haemoglobin, haematocrit. On the other hand, during specific brain monitoring, the brain perfusion (TCD, intracranial pressure), the electrical activity of the brain and sometimes the brain oxygenation (SvjO2) are controlled. The data obtained from the brain monitoring must always be interpretated carefully. A child with a severe head trauma, in ICU, always requires constant and competent medical attention.
