Syndrome de Schnitzler : une cause rare d'urticaire systémique - 01/01/00

Résumé |
Introduction. - Le syndrome de Schnitzler associe des lésions urticariennes, des douleurs ostéoarticulaires et une dysglobulinémie à IgM.
Exégèse. - Nous rapportons le cas d'un homme de 50 ans qui présentait une urticaire touchant le et les membres, évoluant par poussées. S'y assocaient des accès fébriles et des douleurs inflammatoires des genoux et jambes. L'altération de l'état général était modérée. On notait également l'existence d'un syndrome inflammatoire. L'électrophorèse et l'immunoélectrophorèse a mis en évidence une dysglobulinémie IgM Kappa. Les radiographies osseuses ont montré un épaississement des métaphyses tibiales et péronières, donnant un aspect d'hyperfixation à la scintigraphie osseuse. La symptomatologie a régressé sous corticothérapie. Après deux ans d'évolution, l'état général est excellent, sans modification du tableau hématologique. La symptomatologie est jugulée par le traitement par colchicine qui a permis une décroissance de la corticothérapie.
Conclusion. - Ce patient a un syndrome de Schnitzler avec une atteinte osseuse inhabituelle : douleurs périarticulaires et appositions périostées à l'imagerie. Il a par ailleurs favorablement répondu au traitement par corticoïdes et colchicine, ce qui est également rare au cours de ce syndrome. Un suivi prolongé est indiqué en raison du risque évolutif vers une hémopathie.

Introduction. - The Schnitzler's syndrome first described in 1972, associates urticaria, bone pain, and monoclonal IgM gammapathy.
Exegesis. - A 50-year-old man presented symptoms of urticaria restricted to the trunk and lower members, with episodes of fever accompanied by inflammatory pain in the knees and legs. Slight deterioration of his general condition was also observed. Biological findings showed the existence of an inflammatory syndrome. Electrophoresis and immunoelectrophoresis provided evidence for the existence of underlying IgM gammapathy. Bone X-ray demonstrated the presence of tibial and peroneal metaphysis thickening, with hyperfixation on bone scintigraphy. The patient's condition improved after cortisone and colchicine treatment, allowing decrease in coricosteroid doses. Two years later, except for urticaria, clinical features have disappeared and no hematological disorder has been observed.
Conclusion. - This patient presented Schnitzler's syndrome with unusual bone involvement including peri-articular pain and periosteum affixing of the tibias. Therapy with cortisone and colchicine proved to be efficacious, which is also rare in this disease. The syndrome outcome is unforeseeable, often benign. As in the princeps case, hematologic disorders can evolve to lymphocytoma or myeloma, and call for regular follow-up of the patients due to potential occurrence of hematological disease.
