Résumé |
La médiastinite postérieure aiguë ou médiastinite descendante nécrosante reste une pathologie rare. Malgré l'antibiothérapie, les progrès de la réanimation et de la chirurgie, elle est encore responsable d'une mortalité élevée. Nous rapportons un cas de médiastinite par perforation de l'oesophage cervical. Malgré une antibiothérapie et un traitement chirurgical l'état de ce patient restait stationnaire. Une oxygénothérapie hyperbare a été introduite et a permis de rapidement contrôler l'état hémodynamique du patient. L'oxygénothérapie hyperbare, en adjonction au traitement habituel, pourrait améliorer la survie dans cette affection.

In spite of antibiotic treatment, of progress of resuscitation and surgery, acute posterior mediastinitis remains associated to a high mortality. We report a case of man with posterior mediastinitis by perforing of the cervical oesophagus. While his state remained still with the classic treatment, the contribution of the hyperbaric oxygen therapy quickly improved his state. This therapeutics in addition in the usual treatment could improve the survival in this affection.
