Résumé |
Introduction |
La maladie de Kaposi (MK) est une affection multifocale à expression cutanée et viscérale. La MK peut survenir dans un contexte endémique, être associée au virus de l’immunodéficience humaine (VIH) ou compliquer un état d’immunosuppression, notamment d’origine iatrogène chez des sujets transplantés. Le but de ce travail est de tracer le profil épidémiologique, clinique, thérapeutique et évolutif de la MK iatrogène au Maroc en dehors du contexte de la transplantation d’organes.
Méthodes |
Étude rétrospective menée dans le service de dermatologie du centre hospitalier Ibn Sina de Rabat-Maroc, portant sur une période de 21ans et incluant 14 patients présentant une MK iatrogène confirmée histologiquement.
Résultats |
L’âge moyen des patients était de 56ans. Il y avait huit hommes et six femmes. Tous recevaient une corticothérapie, associée au cyclophosphamide dans trois cas et à l’azathioprine dans un cas. Le délai moyen d’apparition des lésions après l’introduction du traitement était de 16,5 mois. Le mode de révélation était cutané dans tous les cas. Une atteinte muqueuse était présente chez 35,7 % des patients et une atteinte viscérale chez un seul. Les sérologies pour le VIH étaient négatives chez tous les patients. La sérologie HHV8 était positive dans 78,5 % des cas. L’attitude thérapeutique reposait principalement sur la diminution ou l’arrêt de l’immunosuppresseur. L’évolution était favorable dans la plupart des cas.
Conclusion |
Au Maroc, la MK est une complication rare mais non exceptionnelle des traitements immunosuppresseurs, notamment la corticothérapie. La maladie s’est révélée par des manifestations cutanées chez tous nos malades, illustrant l’intérêt d’un suivi régulier et d’un examen dermatologique systématique des patients sous immunosuppresseurs, et suggérant l’intérêt d’un dépistage de l’infection HHV8 avant d’introduire de tels traitements.

Background |
Kaposi’s disease (KD) is a multifocal disease affecting the skin and viscera. KD can occur in an endemic setting: it may be associated with human immunodeficiency virus (HIV) or it may occur as a complication of immunosuppression, particularly of iatrogenic origin in transplant patients. The purpose of this study is to describe the epidemiological, clinical and therapeutic profile and the course of iatrogenic KD in Morocco in a setting not involving organ transplantation.
Patients and methods |
A retrospective study conducted at the dermatology department of the Ibn Sina hospital centre in Rabat, Morocco, covering a 21-year period and including 14 patients presenting histologically confirmed iatrogenic KD.
Results |
Eight men and six women were included with a mean age of 56 years. All patients received corticosteroids, in combination with cyclophosphamide in three cases and with azathioprine in one case. The mean time to onset of lesions after the start of treatment was 16.5 months. The presentation in all cases was cutaneous. Impaired mucosal membrane was seen in 35.7% of patients, with visceral involvement being seen in only one patient. HIV serology tests were negative in all patients but HHV8 serology tests were positive in 78.5% of patients. Treatment consisted primarily of reduction or withdrawal of the immunosuppressant. The outcome was favourable in the majority of cases.
Conclusion |
In Morocco, KD is a rare but not exceptional complication of immunosuppressant therapy, particularly corticosteroids. The disease presented as a skin disorder in all of our patients, thus emphasising the value of regular follow-up and routine dermatological examination of patients on immunosuppressant therapy, and suggesting the value of screening for HHV8 infection before initiating such therapy.
