Le thrombus de la crosse de l'aorte : une pathologie rare en réanimation - 01/01/00

Résumé |
Une femme de 54 ans a été admise en réanimation pour intoxication médicamenteuse volontaire au zolipidem (Stilnox®), au dimenhydrinate (Mercalm®), et à l'oestradiol 17 bêta (Oromone®). La scanographie cérébrale réalisée devant des signes cliniques évocateurs de souffrance cérébrale montrait de multiples zones d'ischémie témoignant d'infarctus cérébraux récents. Une échographie transoesophagienne pratiquée 12 heures après mettait en évidence la présence d'un thrombus mobile de la crosse de l'aorte, sans lésions athéromateuses. Une ischémie du membre supérieur droit est apparue secondairement, probablement consécutive à un embole issu du thrombus aortique. La patiente est décédée au troisième jour suite à l'aggravation de l'ischémie cérébrale. Ce cas clinique rappelle que le diagnostic d'intoxication médicamenteuse doit rester un diagnostic d'élimination et que la thrombose de la crosse de l'aorte, pathologie rare en réanimation, doit être évoquée de principe devant un infarctus cérébral si l'on suspecte des emboles d'origine artérielle.

Abstract |
A 54-year-old patient was admitted to the intensive care unit for voluntary drug intoxication with zolipidem (Stilnox®), dimenhydrinate (Mercalm®), and oestradiol 17 beta (Oromone®). Four hours after the admission the patient was comatose. Cerebral computerized tomodensitometry demonstrated multiple zones of ischaemia. Transoesophageal echocardiography was performed 12 hours after the arrival of the patient and revealed a mobile thrombus of the aortic arch. The remainder of the visualized aortic arch did not present atherosclerotic plaque. Secondarily, ischaemia of the right superior limb was diagnosed probably cause by emboli originating in the aortic thrombus appeared. The patient died three days later after her arrival, because of neurologic sequelae of the cerebral embolic events. This clinical case underlines the concept that the diagnosis of drug intoxication must remain a diagnosis of elimination. The thrombosis of the aortic arch is a rare pathology in intensive care units. In the presence of unexplained ischaemic stroke and an peripheral emboli, the thrombosis of the aortic arch should be suspected.
