Tétraplégie au décours d'un coma par intoxication médicamenteuse - 01/01/01

Résumé |
Un cas de tétraplégie complète a été mis en évidence au décours d'un coma toxique chez une patiente présentant une pathologie cervicarthrosique sévère et qui, après l'ingestion volontaire de flunitrazépam, est restée une vingtaine d'heures en position demi-assise, rachis cervical en hyperflexion et rotation. Une perte totale de sensibilité de niveau C4 et une tétraplégie de niveau C6 ont été constatées au moment du réveil, provoqué par l'injection de flumazénil. Les différents examens à visée diagnostique ont éliminé une neuropathie périphérique, un processus compressif, une pathologie infectieuse, une occlusion artérielle. La souffrance médullaire a donc été expliquée par une baisse du flux sanguin médullaire possiblement aggravée par une altération de son autorégulation à l'occasion de la malposition prolongée. Cette souffrance a été favorisée par la pathologie cervicarthrosique qui fragilise la moelle et compromet le flux artériel. Elle a été aggravée par la relative hypotension due à la position assise et par la dépression respiratoire des benzodiazépines. L'évolution a été défavorable : aucune récupération ni sensitive ni motrice, et décès au 13e jour par arrêt cardiaque. Cette complication exceptionnelle au décours d'un coma toxique rappelle la nécessité d'éviter, au bloc opératoire comme en médecine d'urgence, les positions vicieuses prolongées du rachis cervical.

Abstract |
It is reported a case of quadriplegia occurring in a 67-year-old women after she commits suicide with flunitrazepam. The patient stayed during around twenty hours unconscious, in the sitting position, with an extreme flexion of the neck on the left side. After injection of flumazenil the patient's consciousness was restored. Nevertheless a complete sensitive and motor deficit at the C4 and C5 level was then observed. Several diagnosis such as peripheral neuropathy, infectious disease, or arterial occlusion were eliminated and we finally postulated that the regulation of the spinal blood flow had likely been disturbed by the prolonged flexion of the neck, the hypotension and the putative respiratory depression. The fact that the patient suffered from cervicarthrosis would have already impaired the spinal blood flow regulation and consequently had probably damaged the spinal cord. No clinical improvement of the quadriplegia was noticed and the patient died in the intensive care unit thirteen days after admission. Such an exceptional complication after a toxic coma remind us the necessity to avoid long lasting vicious position of the cervical spine in anaesthesia and emergency practice.
