Résumé

Dans la prise en charge du sujet alcoolodépendant en désintoxication, la représentation du futur et son élaboration a une part importante. En effet, la question « comment vivre sans l’alcool ?» est une question fondamentale pour le sujet désirant s’abstenir. Dans l’optique de mieux comprendre le fonctionnement cognitif de la personne alcoolodépendante et afin d’améliorer la prise en charge psychothérapeutique, cette étude a pour objectif de comprendre comment un sujet alcoolodépendant en début de prise en charge évalue émotionnellement son futur. Le matériel utilisé est une liste de 15 mots déterminés comme favorisant l’évocation d’événements de vie. Pour chaque mot, nous avons demandé aux participants d’imaginer un événement futur lui arrivant et de l’évaluer selon quatre variables qui sont : la valence émotionnelle, l’intensité émotionnelle, la probabilité que cet événement se produise et sa netteté. Les résultats montrent que le patient alcoolodépendant imagine moins de situations positives que le sujet non dépendant et autant de souvenirs négatifs. De plus, le patient alcoolodépendant évalue ses situations positives imaginées comme plus intenses et les négatives comme moins intenses par rapport au sujet non dépendant. Ces résultats démontrent qu’il existe un biais d’interprétation des événements de vie chez le patient alcoolodépendant en début de prise en charge. Ce biais a pour conséquence une évaluation émotionnelle spécifique des évènements vie futurs. Jusqu’à présent, nous avions tendance à évaluer le sujet face au stimulus « alcool » et avons basé la prise en charge en thérapie cognitivo-comportementale (TCC) autour du stimulus alcool. Étant donné que notre matériel n’évoque pas l’alcool, nos résultats démontrent que le traitement cognitif du sujet alcoolodépendant biaise de façon générale son jugement sur ses événements de vie. Ce fonctionnement plus général peut être une phase complémentaire de la « prise en compte des situations à haut risque » utilisée dans le modèle de Marlatt et Donovan (Marlatt et Donovan, 2008 [23]) qui est, quant à lui, spécifique à l’objet « alcool ».

Summary

How the future and its development are represented has an important role in the treatment of alcohol dependent subjects during detoxification. Indeed the question “How to live without alcohol?” is fundamental to subjects wishing to abstain. The purpose of this study is to understand how alcohol dependent subjects assess their future emotionally at the start of their treatment, in order to gain better understanding of cognitive functioning of the alcohol dependent person and improve their psychotherapeutic care.

Method

Participants: two groups of 20 subjects were recruited in a psychiatric clinic. The subjects were all hospitalized for anxiety-depressive disorders and/or alcohol dependence. Each group consisted of 11 women and nine men. We matched subjects by age (+ or –2 years) and sex. The first group consisted of 20 participants with alcohol dependence (mean age=42.85, σ=8.91). To be included in this group, participants had to be diagnosed alcohol-dependent according to DSM IV [16] criteria, have an AUDIT score above 13 (Saunders et al., 1993[17]), be recently sober (about seven days after hospitalization) and be aware of their drinking problem (clinical evaluation). The second group, the “Control group”, consisted of 20 non-alcohol dependent participants (mean age=43.45, σ=7.94). To be included in the control group, subjects had to have an AUDIT below 8 (Saunders et al., 1993[17]) score. To ensure that results were not a consequence of a non-equivalent anxious or depressive level, depending on the group, all participants took the BDI (Beck et al., 1961 [18]) and the STAI II (Spielberger et al., 1995 [20]).

Material

The material used was a 15-word list identified as encouraging the recall of life events by Rusinek and al., 1999 [21], (e.g. house, family, animal...). For each word, participants had to imagine themselves in a potential future situation. Each situation, without being mentioned initially, was then rated by the subject on analogue scales of 80mm. The participant simply had to put a cross between two poles on the scale to assess the reported event according to four criteria: emotional valence, emotional intensity, probability of this event occurring and its clarity.

Results

All statistical processing was performed by computer using the Statview©. For Student’s t distribution, we performed a Bonferroni correction and decided to adopt a significance threshold ⍺=0.0167. The results show that alcohol dependent patients imagined fewer positive situations than the non-dependent subject (t38=3.933; P=0.0003) and as many negative memories (t38=0.637; P=0.5281). In addition, alcohol dependent patients assessed imagined positive situations as more intense (t38=2.736; P=0.0094) and sharper (t38=2.834; P=0.0073). In contrast, imagined negative situations were assessed as less intense in comparison with those of a non-dependent subject (t38=2.657; P=0.0116).

Discussion

These results show bias in any interpretation of life events, including beyond the scope of alcohol, by alcohol dependent patients at the start of treatment, especially as levels of depression and anxiety in subjects do not produce any significant results.

This bias results in a specific emotional evaluation of future life events. Alcohol dependent subjects find it less easy to imagine themselves in a positive situation, while intensifying any emotion felt and the clarity of this event. In addition, alcohol dependent subjects have a kind of disaffection faced with imagined negative future situations. This functioning could be a specific mechanism that could favor “the relapse”. Until now, we tended to assess the subject with the stimulus of “alcohol” and have based CBT treatment around this stimulus. Since our material does not mention alcohol, our results indicate that cognitive treatment of the alcohol dependent subject generally skews their judgment on their life events. Thus, this more general functioning could be a complementary phase in the “consideration of high-risk situations” used in the Marlatt and Donovan model (Marlatt and Donovan, 2008 [23]), which is, itself, specific to the object “alcohol”.
