Summary |
Introduction |
Postoperative infection is the most common postsurgical oral complication but no double-blind, randomized, placebo-controlled trial has been made yet to assess amoxicillin prophylaxis for oral surgery. The aim of this pilot study was to assess the effectiveness of prophylactic amoxicillin 3g per os versus placebo in the prevention of postoperative oral infections.
Methods and materials |
A 5-year, multicentric, randomized, double-blind clinical trial was made on healthy patients undergoing complex oral surgery. Final analysis was by intention to treat.
Results |
The 286 patients were randomly assigned to the amoxicillin or placebo group. After a mean follow-up of two weeks, 11 patients in the control group and three patients in the amoxicillin prophylaxis group developed a postoperative infection (P=0.001 in unilateral test). The hazard ratio was 0.27 [95% CI: 0.07-0.94].
Conclusion |
Amoxicillin prophylaxis seems to be effective in preventing postoperative infection in complex oral surgery, especially when mandibular third molars and multiple avulsions are involved.

Introduction |
Les infections postopératoires sont les complications les plus fréquentes en chirurgie buccale. Aucune étude randomisée en double insu n’a évalué l’intérêt de l’antibioprophylaxie par amoxicilline dans le cadre de la chirurgie buccale. L’objectif de notre étude pilote versus placebo a été d’évaluer l’efficacité prophylactique de l’administration de 3g d’amoxicilline, per os, en chirurgie buccale.
Matériel et méthodes |
Cette étude clinique, randomisée en double insu, multicentrique, sur cinq ans, a inclus des patients en bonne santé candidats à une chirurgie buccale complexe.
Résultats |
Les 286 patients ont été répartis de manière aléatoire dans les groupes « amoxicilline » ou placebo. Après un suivi moyen de deux semaines, 11 patients du groupe témoin et trois patients du groupe « amoxicilline », ont développé une infection postopératoire (p=0,001 en test unilatéral). Le hazard-ratio était de 0,27 [95 % CI : 0,07-0,94].
Discussion |
La prophylaxie par amoxicilline semble efficace dans la prévention des infections postopératoires en chirurgie buccale complexe, en particulier lors d’avulsions multiples ou d’avulsions de dents de sagesse mandibulaires.
