Hyperparathyroïdie maligne compliquée de calcifications diffuses avec atteinte pulmonaire chez un patient hémodialysé chronique - 02/12/12

Les calcifications métastatiques représentent une complication parfois sévère de l’insuffisance rénale chronique terminale. L’atteinte pulmonaire est le plus souvent asymptomatique, mais son évolution peut être fatale. Nous rapportons l’histoire clinique d’un patient hémodialysé chronique qui a développé une hyperparathyroïdie « maligne » en raison d’une mauvaise compliance thérapeutique. L’évolution a été marquée par l’apparition de calcifications métastatiques vasculaires et viscérales avec une atteinte pulmonaire qui a conduit au décès. La persistance d’un produit phosphocalcique élevé a certainement favorisé cette évolution.

Metastatic calcifications are a potentially severe complication of end-stage renal disease. In the lungs, they are often asymptomatic with a possible lethal evolution. We report the clinical case of a chronic hemodialysis patient who died from extensive metastatic calcifications both vascular and visceral specially localized in the lungs. This evolution was secondary to non compliance of the patient to the treatment of a malignant hyperparathyroidism with an important elevation of phosphocalcium product.
