Summary |
The determination of Human Epithelial receptor 2 (HER2) status by immunohistochemistry (IHC) is very important for diagnosis since the appearance of Trastuzamab in adjuvant and neoadjuvant breast cancer. The objective set for this study was to evaluate the overexpression of HER2 in 160 patients with invasive breast cancer, because of the involvement of erbB family receptors (epidermal growth factor receptor) in breast oncogenesis. The assessment of HER2 status is semi-quantitative, and is following a defined scale: score 1+(negative), score 2+(verification of gene amplification by FISH or CISH more objective and quantitative IHC), score 3+(positive). Our strategy is purely and prognosis is to redo the IHC scores of 2, once you realize the score confirmed a FISH (fluorescence in situ hybridization), and 50 patients have undergone evaluation of HER2 gene amplification/neu. The indication for treatment Herceptin® scores for IHC and FISH amplified 3. The majority of cases are invasive ductal carcinomas: 84.37% of cases. Our results were as follows: 22.25% are HER2 +1, 31.25% HER2 +2, 46.25% HER2 +3. A monoclonal antibody against the HER2 receptor, trastuzumab (Herceptin®) is used as a treatment for women with HER2+. Our case series, although limited to permits to handle patients scoring 2, which in itself is a first and a big step for our laboratory.

La détermination du statut Human Epithelial Réceptor 2 (HER2) par immuno-histochimie (IHC) est très importante pour le diagnostic depuis l’apparition du Trastuzamab dans le traitement adjuvant et néoadjuvant du cancer du sein. L’objectif assigné à ce travail est d’évaluer la surexpression du récepteur HER2 chez 160 patientes atteintes de cancer du sein infiltrant, en raison de l’implication des récepteurs de la famille epidermal growth factor receptor (erbB) dans l’oncogenèse mammaire. L’évaluation du statut HER2 est semi-quantitative, et se fait suivant une échelle définie par : score 1+ (négatif), score 2+ (vérification de l’amplification génique par la technique FISH (hybridation in situ en fluorescence), ou hybridation in situ chromogenic (CISH) plus objectives et quantitatives que l’IHC), score 3+ (positif). Notre stratégie est purement pronostique et consiste à refaire l’IHC des scores 2, une fois le score confirmé on réalise une FISH. Ainsi 50 patientes ont fait l’objet d’évaluation d’amplification du gène HER2/neu. L’indication du traitement Herceptin® concerne les scores IHC 3 et FISH amplifié. La majorité des cas traités sont des carcinomes canalaires infiltrants, soit 84,37 % des cas. Nos résultats ont été les suivants : 22,25 % sont HER2 +1 ; 31,25 % HER2 +2 ; 46,25 % HER2 +3. Un anticorps monoclonal dirigé contre le récepteur HER2, le trastuzumab (Herceptin®) est utilisé comme traitement pour les femmes HER2+. Notre série de cas, bien que limitée à permis de prendre en charge les patientes de score 2, ce qui en soit est une première et un grand pas pour notre laboratoire.
