Il existe actuellement plusieurs antifongiques systémiques appartenant à quatre classes pharmacologiques, chaque famille possédant un mode d’action particulier. L’amphotéricine B agit en se fixant sur l’ergostérol, ce qui conduit à la formation de pores dans la membrane. La 5-fluorocytosine inhibe la synthèse d’ADN et la synthèse protéique. Les azolés inhibent la synthèse de l’ergostérol et les échinocandines inhibent la synthèse des 1-3-beta-glucanes de la paroi.
La résistance microbiologique des Candida spp. aux antifongiques peut être détectée au laboratoire par des techniques commercialisées et des techniques de référence sont également disponibles.
La résistance à l’amphotéricine B reste exceptionnelle et peut être liée à une disparition de l’ergostérol membranaire. La résistance à la 5-fluorocytosine peut apparaître rapidement et peut être due à un défaut de pénétration intracellulaire ou à un défaut de transformation en 5-fluorouracil qui est la molécule active. La résistance aux azolés peut être liée à des mécanismes variés qui peuvent s’associer chez une souche donnée. On en distingue quatre grands groupes qui incluent (i) des modifications de la cible (la 14-alpha-déméthylase) par mutation du gène codant pour l’enzyme, (ii) une surproduction de la cible, (iii) des phénomènes d’efflux, par surproduction de transporteurs, (iv) des altérations d’autres étapes de la voie de synthèse de l’ergostérol. La résistance aux échinocandines est liée à une modification de la cible par mutations des gènes fks qui codent pour la beta-1-3-D-glucane synthase.

There are currently several antifungal drugs belonging to 4 pharmacological families, and each family exhibit a specific mechanism of action. Amphotericin B binds to ergosterol leading to the formation of transmenbrane pores. Flucytosine inhibits DNA synthesis and protein synthesis. Azoles inhibit ergosterol synthesis and echinocandins inhibit 1-3-beta-glucan synthesis, a major component of the cell wall.
Antifungal resistance of Candida spp. can be accurately detected in the clinical microbiology laboratory by commercialized techniques and reference methods are also available.
Amphotericin B resistance remains uncommon and could be related to a lack of ergosterol in the fungal membrane. Flucytosine resistance can emerge rapidly and can be due to a defect of intracellular penetration or to a defect of the metabolism of the drug into fluorouracil which is the active compound. Several mechanisms can be responsible for azole resistance and these mechanisms can be combined in a given isolate. There are four main groups of azole resistance mechanisms including (i) modification of the target (14-alpha-demethylase) by mutation of the gene coding for this enzyme, (ii) overproduction of the target, (iii) extracellular efflux by overproduction of efflux pumps, (iv) modifications of other steps of the ergosterol biosynthesis pathway. Echinocandin resistance is due to modification of the target enzyme (beta-1-3-D-glucane synthase) by mutation of the fks genes.
