Liver transplantation eliminates insulin needs of a diabetic patient - 17/02/08

We report the case of a diabetic patient whose insulin needs disappeared after liver transplantation. Non insulin-dependent diabetes mellitus was diagnosed when she was 47, and was treated by hypoglycemic drugs and then insulin. Chronic post-hepatitis C cirrhosis was diagnosed at the age of 55 and required liver transplantation 2 years later. During the postoperative course, the insulin doses required to maintain normal glucose levels progressively decreased, and insulin became completely unnecessary by the 29 th postoperative day. After insulin was stopped, glucose levels remained within normal ranges for the 5-year-long follow-up, despite the worsening of a preexisting diabetic nephropathy and the occurrence of a diabetic retinopathy. This case highlights the fact that liver transplantation may eliminate insulin needs in a diabetic patient but also shows that degenerative complications may occur despite apparent remission of diabetes.

La transplantation d'organe peut déclencher ou aggraver un diabète pré-existant. Nous rapportons le cas d'une patiente diabétique dont les besoins en insuline ont disparu après transplantation hépatique. Un diabète non-insulinodépendant a été diagnostiqué à l'âge de 47 ans et fut traité par antidiabétiques oraux puis par insuline. Une cirrhose chronique post-hépatite C fut diagnostiquée à l'âge de 55 ans et nécessita une transplantation hépatique 2 ans plus tard. En post-opératoire, les doses d'insuline nécessaires pour maintenir l'équilibre glycémique ont progressivement diminué et l'insulinothérapie a été interrompue au 29 e jour post-opératoire. Un suivi de 5 ans post-greffe a montré la normalisation prolongée de la glycémie en l'absence de traitement par insuline ou antidiabétiques oraux. Une aggravation d'une néphropathie diabétique pré-existante et l'apparition d'une rétinopathie diabétique ont cependant été constatées. Cette observation souligne qu'une disparition des besoins en insuline peut être obtenue après transplantation hépatique mais qu'elle n'empêche pas la survenue des complications dégénératives.
