Introduction |
L’interface endothélio-descemetique (IED) d’une kératoplastie lamellaire antérieure profonde (KLAP) peut faire le lit d’une prolifération néovasculaire. Le bevacizumab (avastin) est une alternative anti-angiogénique disponible pour le traitement curatif des néovaisseaux cornéens (NVC) de localisation profonde.
Observation |
Nous rapportons le cas d’une femme de 49ans qui, après avoir bénéficié d’un traitement d’astigmatisme par incisions relaxantes sur KLAP, a présenté un épisode de rejet stromal associé à une invasion néovasculaire importante de l’IED, donneur-receveur. Deux injections sous-conjonctivales de bevacizumab à un mois d’intervalle ainsi qu’une cautérisation du tronc néovasculaire nourricier furent nécessaires et efficaces. Il n’a pas été constaté de récidive après deux années de surveillance.
Discussion |
L’appel néovasculaire dans l’IED NVC peut être une cause du rejet de greffe pré-descemetique. L’association d’agents anti-angiogéniques et de moyens physiques permet de juguler le rejet de greffe et de restituer une bonne fonctionnalité, durablement.

Introduction |
The endothelio-descemetic interface (EDI) of a deep anterior lamellar keratoplasty (DALK) may become the locus of a neovascular proliferation. Bevacizumab (Avastin) is an antiangiogenic alternative available for curative treatment in deep corneal neovascularization.
Observation |
We report the case of a 49-year-old woman who had received two arcuate relaxing incisions for high astigmatism after DALK. She developed graft rejection associated with significant neovascular stromal invasion in the EDI of the graft-host interface. Two subconjunctival injections of bevacizumab 1month apart as well as thermocauterization of the feeding vessel were necessary and effective. No recurrence has been observed after 2years of follow-up.
Discussion |
Corneal neovascularization can be a cause of DALK rejection. A combination of anti-antiangiogenic and physical methods may abort graft rejection and permanently restore graft function.
