Évaluation de l’analyseur d’hématologie Beckman Coulter® HmX™ dans le centre hospitalo-universitaire d’Oran - 08/11/13

Résumé |
Introduction |
Le choix d’un automate d’hématologie est une étape déterminante, qui doit prendre en compte à la fois la qualité des résultats et des impératifs économiques : charge de travail, structure et organisation du laboratoire.
Patients et méthodes |
C’est dans cet esprit que nous avons évalué pendant une période de trois mois l’analyseur d’hématologie : le HmX™ Coulter avec passeur d’échantillons de la société Beckman Coulter. Cet automate réalise la numération sanguine, la formule leucocytaire et la quantification des réticulocytes. Dans un premier temps, nous avons évalué les caractéristiques propres de l’appareil. Dans un second temps, nous avons évalué la pertinence, la sensibilité et la spécificité des alarmes en comparaison avec la méthode de référence qui est la microscopie optique. Pour cela, 125 frottis sanguins provenant de service d’hématologie et de réanimation ont été examinés en microscopie optique. Les tests techniques ont été réalisés selon les recommandations du Comité international pour l’évaluation d’automates d’hématologie.
Résultats |
Les performances analytiques ont été satisfaisantes en particulier le grand intervalle de linéarité et l’absence de contamination. Concernant les alarmes : le taux de rejet est de 63 %, la sensibilité 86 %, la spécificité 70 %, la valeur prédictive positive 80 %, la valeur prédictive négative 78 % et l’efficacité 80 %. Les alarmes myélémie et lymphocytes atypiques n’ont jamais été sources de faux négatifs. Les alarmes érythroblastes et agrégats plaquettaires n’ont pas engendré de faux positifs. L’alarme blaste était responsable d’un seul cas de faux négatif.
Conclusion |
La fidélité de l’automate est satisfaisante : l’absence de contamination, le grand intervalle de linéarité pour les leucocytes, les hématies et les plaquettes ainsi qu’une bonne pertinence des alarmes par rapport aux anomalies trouvées sur le frottis sanguin périphérique. Du point de vue convivialité et praticabilité, le HmX™ Coulter a été vivement apprécié.

Introduction |
The choice of an automaton of haematology is a determining stage, which has to take into account at the same time the quality of the results and the economic imperatives: workload, structure and organization of the laboratory.
Materiel and methods |
It is in this spirit that we estimated during a period of 3months the analyzer of haematology: the HmX™ Coulter with boatman of samples of the company Beckman. This automaton realizes the blood numeration, the formula leukocytic and the reticulocyte count. At first, we estimated the appropriate characteristics of device. Secondly, we estimated the relevance, the sensibility and the specificity of the alarms by comparing with the reference method, which is the optical microscopy. For that purpose, 125 blood smears resulting from service of haematology and from resuscitation were examined in optical microscopy. The technical tests were realized according to the recommendations of the International committee for evaluation of automatons of haematology.
Results |
The analytical performances were satisfactory in particular the big interval of linearity and the absence of contamination. As regards the evaluation of the alarms system: rate of rejection is 63%, the sensibility 86%, the specificity 70%, the positive predictive value 80%, the negative predictive value 78% and the efficiency 80%. The alarms myelaemia and atypical lymphocytes were never sources of false negatives. The alarms erythroblasts and platelet aggregates did not engendered positive forgery. The blast cell alarm was responsible for a single case of false negative.
Conclusion |
The faithfulness of automaton is satisfactory: the absence of contamination, the big interval of linearity for the leukocytes, the red blood cells and the platelets as well as a good relevance of the alarms with regard to the anomalies found on the peripheral blood smear. From the user-friendliness and practicability point of view, the HmX™ Coulter was deeply appreciated.
