Abstract |
Objective |
To systematically investigate chemotherapy versus chemotherapy followed by involved field radiotherapy for limited-stage aggressive non-Hodgkin's lymphoma.
Method |
A systematic literature search of Pubmed, EMBASE, and the Cochrane central register of controlled trials was performed. Data from all randomized controlled trials comparing chemotherapy alone with chemotherapy followed by involved field radiotherapy in patients with limited-stage aggressive non-Hodgkin's lymphoma was collaborate. The primary outcome was overall survival; secondary outcomes were event-free survival and cumulative incidences of toxicity.
Results |
Three randomized controlled trials, with 1263 participants in total, were analyzed. Median follow-up was 11 years. Patients receiving chemotherapy followed by involved field radiotherapy had a significant benefit on event-free survival (hazards ratio [HR]=1.47, 95% confidence interval [CI]: 1.14 to 1.90; P=0.003), but not improved overall survival (HR=1.00, 95% CI: 0.80 to 1.25; P=0.978) at 8years, in relation to those in the chemotherapy alone arm. There were great differences in the reporting of cumulative incidences of toxicity.
Conclusion |
Chemotherapy followed by involved field radiotherapy, as compared with chemotherapy alone, is associated with better event-free survival, but no survival benefit is observed for patients for limited-stage aggressive non-Hodgkin's lymphoma. These results needed to be confirmed by high quality trials and further studies in the East.

Évaluer la chimiothérapie par rapport à la chimiothérapie suivie d’une radiothérapie pour les lymphomes non hodgkiniens d’extension limitée agressifs.
Matériels et méthodes |
Une recherche systématique de la littérature sur Pubmed, EMBASE et Cochrane Central Register of Controlled Trials a été effectuée. Les données de tous les essais contrôlés randomisés comparant la chimiothérapie seule et une chimiothérapie suivie d’une radiothérapie de type involved field chez les patients atteints de lymphome non hodgkinien d’extension limitée agressif à un stade limité ont été prises en compte. Le critère principal était la survie globale, les critères d’évaluation secondaires la survie sans évènement et les incidences cumulatives de toxicité.
Résultats |
Trois essais randomisés contrôlés, avec 1263 participants au total, ont été analysés. Le suivi médian était de 11ans. Les patients recevant une chimiothérapie suivie d’une radiothérapie de type involved field avaient un bénéfice significatif en termes de survie sans évènement (risk ratio [HR]=1,47, intervalle de confiance à 95 % [IC] : 1,14 à 1,90, p=0,003), mais non survie globale (HR=1,00, IC 95 % : 0,80 à 1,25, p=0,978) de la huitième année, par rapport à ceux ne recevant que la chimiothérapie. Il y avait une différences importante d’incidence cumulée de toxicité.
Conclusion |
La chimiothérapie suivie d’une radiothérapie de type involved field, par rapport à la chimiothérapie seule, est associée à une meilleure probabilité de survie sans évènement, mais aucun avantage de survie n’a été observé chez les patients atteints de lymphome non hodgkinien d’extension limitée agressif. Ces résultats devaient être confirmés par des essais de haute qualité et d’autres études en occident.
