Bispectral index transiently decreased to “0” during per-embolization rupture of an intracranial aneurysm - 18/02/14
L’index bispectral diminue transitoirement à « 0 » pendant la rupture d’un anévrysme intracrânien au cours de la procédure d’embolisation

Interpreting a Bispectral Index (BIS) of “0”, corresponding to an isolelectric electroencephalography, can be difficult. After ruling out technical issues, such as leads disconnection, several possible causes for a decrease in the BIS persists, including deep anesthesia, hypothermia, decrease in the cerebral perfusion pressure and cerebral ischemia. We report a sudden transient decrease of the BIS to “0” in a patient that underwent a coil embolization of a ruptured intracranial aneurysm and suggest that the change in BIS values could provide useful information about the cerebral hemodynamic during aneurysm treatment and might provide indications of a serious cerebral event.

Il peut être difficile d’interpréter un index bispectral à « 0 », qui correspond à un électroencéphalogramme isoélectrique. Après avoir exclu les raisons techniques, telle que la déconnexion, plusieurs causes possibles persistent pour expliquer la baisse de l’index bispectral: une anesthésie profonde, l’hypothermie, une diminution de la perfusion cérébrale ou une ischémie cérébrale. Nous décrivons un cas clinique où l’index bispectral s’est temporairement abaissé à « 0 » lors d’une rupture d’anévrisme intracrânien pendant la procédure d’embolisation.
