Résumé |
Objectifs |
Les résultats de l’ACOSOG Z0011 ont remis en cause l’utilité du curage axillaire (CA) complémentaire en cas de ganglion sentinelle (GS) métastatique. L’objectif de notre étude a été d’évaluer l’impact non seulement de l’omission du CA complémentaire selon les critères d’inclusion de l’étude ACOSOG Z0011 en cas de GS métastatique mais aussi des conséquences de l’application d’un nouveau référentiel de prise en charge du cancer du sein sur les modalités du traitement complémentaire.
Patientes et méthodes |
Cette étude rétrospective a inclus entre novembre 2007 et janvier 2012 les patientes avec cancer du sein T1-T2N0 avec GS métastatique répondant aux critères de renoncement au CA selon l’étude de l’ACOSOG Z0011. Les patientes ont été présentées, de façon anonyme et aléatoire, en réunion de concertation multidisciplinaire (RCP) à 3 reprises : avec l’information complète comprenant le CA (RCP1) ; avec l’information du GS seul (RCP2) ; et avec l’information complète du CA selon les protocoles en vigueur en 2013 (RCP3). Lors de chaque présentation, nous avons recueilli les différents traitements adjuvants proposés : chimiothérapie, hormonothérapie, radiothérapie (avec champs d’irradiation). Nous avons ensuite comparé les propositions thérapeutiques des 3 présentations.
Résultats |
Cinquante-huit patients étaient éligibles aux critères d’inclusion de l’ACOSOG Z0011. Les traitements effectivement proposés en RCP1 comportaient : 94,8 % de chimiothérapie, 77,6 % de radiothérapie du sein et des aires ganglionnaires et 91,4 % d’hormonothérapie. Lors de la RCP2, il n’a pas été observé de différence significative par rapport aux traitements proposés lors de la RCP1 avec des taux de chimiothérapie, radiothérapie et hormonothérapie, respectivement, 89,7, 79,3 et 91,4 %. Lors de la RCP3, il a été mis en évidence une diminution significative des indications de chimiothérapie (82,8 % ; p=0,03) et de radiothérapie du sein et des aires ganglionnaires (56,9 % ; p=0,02) par rapport aux propositions thérapeutiques lors de la RCP1.
Discussion et conclusion |
L’absence d’information du CA ne semble pas modifier significativement les indications des traitements adjuvants. En revanche, l’évolution de nos référentiels entraîne à lui seul une diminution des traitements adjuvants.

Objectives |
The results of the ACOSOG Z0011 questioned the usefulness of axillary lymph node dissection (ALND) in case of metastatic sentinel lymph node (SLN). The aim of our study was to assess the impact of the omission of ALND according to the inclusion criteria of the ACOSOG Z0011 study if SLN are metastatic but also the consequences on prescription of the application of a new standard of care for adjuvant treatment.
Patients and methods |
This retrospective study included, between November 2007 and January 2012, patients with T1-T2N0 breast cancer and metastatic SLN meeting the criteria for omission of completion ALND according to the study of the ACOSOG Z0011. Patients were submitted anonymously and randomly in multidisciplinary meeting (MM) 3 times: with complete information including ALND (MM1), with information from SLN alone (MM2) and with complete information of ALND according to the current protocols in 2013 (MM3). During each presentation, we collected the decision of the different adjuvant treatments proposed: chemotherapy, hormonal therapy, radiotherapy (with radiation fields). Then, we compared therapeutic proposals of the 3 presentations.
Results |
Fifty-eight patients were eligible for inclusion criteria of the ACOSOG Z0011. Treatments actually proposed during MM1 consisted of 94.8 % of chemotherapy, 77.6 % of breast and lymph nodes radiotherapy and 91.4 % of hormone therapy. During the MM2, there was no significant difference compared to the decision taken during MM1. In fact, during MM2, we decided chemotherapy, radiotherapy and hormonotherapy respectively in 89.7, 79.3 and 91.4 % of the cases. During the MM3, it was shown a significant decrease in the indications of chemotherapy (82.8 %, P=0.03) and lymph nodes irradiation (56.9 %, P=0.02) compared to the therapeutic proposals of the MM1.
Discussion and conclusion |
The lack of information of ALND does not seem to significantly alter indications for adjuvant treatment. Otherwise, the evolution of our references causes a decrease in adjuvant therapy.
