Objectif |
Étudier, à partir d'une série de 14 cas et d'une revue de la littérature, la place du drainage trans-fistulo-oddien à ciel fermé dans le traitement des kystes hydatiques du foie.
Méthodes |
De 1985 à 2000, 14 patients avec un kyste hydatique largement ouvert dans les voies biliaires ont été traités par drainage trans-fistulo-oddien sans ouverture directe de la cavité kystique. La cholédoscopie a confirmé l'absence de résidus hydatique dans la cavité résiduelle dans tous les cas. La cavité résiduelle a été drainée à partir d'un drainage externe du cholédoque type Kehr dans tous les cas. Dans quatre cas, un deuxième kyste hydatique du foie a été traité dans le même temps opératoire.
Résultats |
Le drainage interne trans-fistulo-oddien a été de réalisation facile dans tous les cas sans incident. Cependant, deux complications post-opératoires spécifiques à la technique ont été notées : une fistule biliaire externe dans un cas et une surinfection de la cavité résiduelle compliquée de septicémie et responsable du décès malgré la reprise opératoire dans un autre cas.
Conclusion |
Le drainage interne trans-fistulo-oddien à ciel fermé est indiqué pour les kystes hydatiques centro-parenchymateux de diamètre inférieur à 15 cm, à périkyste souple, et compliqués d'une large fistule kysto-biliaire supérieure ou égale à 5 mm. Le contrôle cholédoscopique peropératoire de la cavité kystique est obligatoire.

Objective |
The purpose of this study was to examine the contribution of internal trans-fistulary drainage without opening the cystic cavity for the treatment of hydatic cyst of the liver.
Patients and methods |
Between 1985 and 1990, 14 patients with hydatid cyst of the liver communicating with the bile duct were treated by internal trans-fistulary drainage without opening the hydatic cavity. Choledocoscopy demonstrated good bile flow without a remaining cystic fragment in all cases. The residual cavity was drained via an external drain in the common bile duct. A second hydatic cyst of the liver was also treated in 4 patients.
Results |
Internal trans-fistulary drainage was achieved easily. There were two treatment-related postoperative complications : bilary leakage in one patient and infection of the residual cavity leading to septicemia and death in another.
Conclusion |
Internal trans-fistulary drainage without opening the hydatic cavity is indicated for central hydatic cyst of the liver measuring less than 15 cm with a flexible pericyst and a large biliocystic fistula measuring more than 5 mm. Peroperative choledochoscopy of the hydatic cavity is necessary.
