Nous décrivons un cas d’anaphylaxie à l’amoxicilline 31ans après le dernier traitement par pénicilline chez un patient âgé de 53ans. Il décrivait, 10minutes après la prise du premier comprimé, une réaction anaphylactique de grade II (selon Ring et Messner). Les explorations allergologiques confirmaient une hypersensibilité immédiate allergique à l’amoxicilline. La réaction récente témoigne d’un taux d’IgE anti-amoxicilline suffisant pour déclencher une anaphylaxie et pose donc la question du maintien dans le temps d’une immunité spécifique. Plusieurs hypothèses peuvent expliquer ce phénomène. La sécrétion d’anticorps par les plasmocytes a pu perdurer des années après la stimulation antigénique. Le patient a pu être exposé de manière fortuite aux pénicillines. Des expositions occultes par apport de l’antigène dans l’alimentation ont aussi été rapportées.

We report a case of anaphylaxis in a 53-year-old patient 31years after the last contact with amoxicillin. He had an anaphylactic reaction grade II (according to Ring and Messner) 10minutes after ingesting an amoxicillin tablet. The allergy workup confirmed immediate allergic sensitivity to amoxicillin. This recent reaction suggests the persistence of a sufficient amount of amoxicillin-specific IgE to produce anaphylaxis and raises the question of the possibility of long-term maintenance of amoxicillin-specific sensitivity. Several hypotheses may explain this phenomenon: antibody secretion by plasma cells might last for years after antigenic stimulation, or the patient could have been exposed incidentally to penicillins. Occult exposure by intake of antigens in food has also been reported.
