Introduction |
L'objet de cette étude était de valider un protocole d'extraction dentaire chez un patient sous A.V.K. sans modification de son traitement lorsque l'I.N.R. était inférieur ou égal à 2.8.
Matériel et méthode |
104 extractions (96 patients) ont été réalisées et contrôlées pendant 9 mois chez des patients traités par des A.V.K. Lorsque l'I.N.R. était inférieur ou égal à 2,8, l'extraction était pratiquée. Sinon le traitement était modifié jusqu'à obtention du taux désiré.
Les extractions ont été réalisées sous anesthésie locale para-apicale et l'alvéole suturée sur une gaze hémostatique.
Résultats |
3 patients ont présenté des saignements post-opératoires qui ont nécessité une révision alvéolaire avec application d'acide tranéxamique localement et dans un cas utilisation de colle biologique.
Discussion |
Le protocole suivant est proposé : extraction sous anesthésie locale en ambulatoire avec dosage de l'I.N.R. la veille ; intervention si l'I.N.R. est inférieur ou égal à 2,8 avec suture sur gaze hémostatique et utilisation d'acide tranexamique si des suintements hémorragiques persistent.

Background |
The purpose of this study was to validate a protocol for dental extraction in patients taking oral anticoagulants without changing the treatment when the INR is lower than 2.8.
Material and methods |
One hundred four extractions (96 patients) were performed during a 9 month period in patients taking oral anticoagulants. Extraction was performed when the international normalized ratio (INR) was less than 2.8, otherwise, the treatment was modified until the desired INR was reached. Extractions were performed under para-apical and alveolar local anesthesia and sutured with hemostatic gauze.
Results |
Three patients developed postoperative bleeding requiring alveolar revision with local application of tranexamic acid and in one case use of a biological glue.
Discussion |
The following protocol can be proposed: ambulatory extraction under local anesthesia with an INR the day before extraction: the extraction is performed if the INR is <= 2.8 using hemostatic gauze suture and tranexamic acid in case of persistent bleeding.
