Prédictibilité de la distraction du maxillaire supérieur avec distracteur trans-sinusien |
Introduction |
Ce travail a évalué un nouveau distracteur maxillaire dont le système d'activation est introduit dans le sinus maxillaire.
Patients et méthode |
Quatorze patients âgés de 8 à 55 ans porteurs d'une retrusion maxillaire et médiofaciale importante ont été traités par distraction avec une paire de distracteurs de type maxillaire trans-sinusien (TS-MD développé par l'auteur avec la firme Martin Medizin, Tuttlingen, Allemagne) par voie orale, chacun positionné d'un côté du maxillaire. Le vecteur de distraction a été prédit par un logiciel spécifique (Medicim® N. V., Belgique), et a été adapté au patient par l'intermédiaire d'un modèle stéréolithographique. Une ostéotomie de type Lefort I haute a été réalisée.
Résultats |
L'avancement obtenu a varié de 7 à 22 mm selon les patients. La formation d'os a abouti à une cicatrisation complète de part et d'autre des sections, et à une élongation des tissus mous. Le mouvement prédit du maxillaire dans les trois dimensions a été obtenu. Les distracteurs ont permis un mouvement de rotation vers le bas afin de corriger un open-bite dans tous les cas traités. Les distracteurs ont pu être laissés en place autant de temps que nécessaire jusqu'à consolidation, en général deux mois, afin de prévenir le recul secondaire du fragment osseux distracté.
Discussion |
Les avantages du distracteur TS-MD trans-sinusien sont les suivants :
– la partie la plus volumineuse est positionnée en intra-sinusien, ce qui le rend plus tolérable ;
– l'implantation se fait exclusivement par voie orale, ce qui n'interfère pas avec la vie sociale du patient ;
– il n'est pas nécessaire de réaliser de fixation alvéolo-dentaire.
Le distracteur TS-MD permet également d'éviter la greffe osseuse lorsque l'avancement est important. Il est critique de bien positionner le vecteur, mais un rattrapage d'un côté sur l'autre est également possible.

Introduction |
This work was conducted to evaluate a new maxillary distractor with an activating system introduced into the maxilary sinus.
Patients and method |
Fourteen patients age 8 to 55 years with severe maxillary and midfacial hypoplasia were treated in our center. A trans-sinusoidal maxillary distractor (TS-MD, developed by Dr. N. Nadjmi in cooperation with Martin, Tuttlingen, Germany) was placed intra-orally at each side of the maxilla. The distraction vector was predicted with devoted software (Medicim N.V Belgium), and was transferred to the patients using stereolithographic models and individual templates. A high Lefort I type osteotomy was performed.
Results |
Forward maxillary movement at distractor level varied from 7 to 22 mm. Soft and hard tissue formation resulted in complete healing across the distraction gaps. The maxillary movements and new bone formation in the sagital, horizontal, and vertical planes could be predicted and achieved. Distractors were designed to allow clockwise rotation of the maxilla during the distraction phase allowing the correction of the anterior open-bite in all patients with this skeletal deformity. The distractors were almost completely submerged, and could be left in place as long as necessary to avoid relapse.
Discussion |
The TS-MD distractor has the advantage of good tolerance since the most voluminous part is placed within the maxiallary sinus. Implantation via an exclusively oral approach is a further advantage. The devise does not interfere with the patient's social life and does not require alveolodental fixation. The TS-MD distractor can also aleviate the need for bone grafting in cases with major avancement. Correct position of the vector is crucial but one-sided catch-up distraction is possible.
