Introduction |
L'ostéomyélite temporomandibulaire est une complication rare de l'otite externe nécrosante progressive, survenant souvent après un traitement initial inapproprié de l'affection causale.
Observation |
Un patient âgé de 65 ans, diabétique, a présenté une ostéomyélite temporomandibulaire droite suite à une otite nécrosante progressive. L'atteinte articulaire a été suspectée cliniquement et confirmée par l'imagerie. L'évolution a été favorable sous traitement médical.
Discussion |
L'ostéomyélite temporomandibulaire est une complication inhabituelle de l'otite externe nécrosante progressive. La diffusion de l'infection vers l'articulation temporomandibulaire peut être secondaire à une ostéite du tympanal ou se faire à travers des déhiscences congénitales du conduit auditif externe. Les douleurs, la limitation de l'ouverture buccale et la tuméfaction prétragienne en sont les principaux signes cliniques.
Le traitement est essentiellement médical, basé sur une antibiothérapie.

Osteomyelitis of the temporomandibular joint is a rare complication of invasive external otitis, often occurring after inappropriate initial treatment of the causal affection.
Observation |
A 65-year-old diabetic man was treated for invasive otitis externa complicated by destructive osteomyelitis of the ipsilateral temporomandibular joint. The diagnosis was suspected clinically and confirmed by imaging. The course was favorable with medical treatment.
Discussion |
Osteomyelitis is an extremely unusual complication of invasive otitis externa. Diffusion of the infection toward the temporomandibular joint can be secondary to an osteitis of the bony canal or spread via congenital dehiscences of the external auditory canal. Pain, limitation of mouth opening and auricular swelling are the main clinical signs. Treatment is essentially medical, based on adapted antibiotic therapy.
