La sexualité du post-partum. Analyse du vécu dans le couple noir africain - 20/02/15

La période du post-partum suscite beaucoup d’interrogations dans le couple notamment en matière de sexualité.
Objectif |
Analyser le vécu de la sexualité des couples noirs africains dans le post-partum.
Patientes et méthode |
Il s’agissait d’une étude de type attitudes et connaissances pratiques qui s’est déroulée dans le service de gynécologie-obstétrique et d’andrologie du CHU de Bouaké sur la période allant 30 septembre 2012 au 30 avril 2013. L’étude s’est déroulée en deux étapes. La première étape avait consisté en un pré-test qui a permis de valider le questionnaire qui comportait 16 items. La seconde étape a consisté à le soumettre directement aux patientes, en consultation postnatale, qui avaient accepté de participer à l’étude après un consentement éclairé. Les données ont été analysées à l’aide du logiciel épi-info version 2002.
Résultats |
Sur 395 patientes enquêtées, nous avons noté la reprise des rapports sexuels pour 140 patientes (34,2 %). La reprise s’est faite à l’initiative du partenaire dans 67,9 % des cas, notamment après le retour des couches (53,8 %). Elle s’expliquait par le souci de satisfaire le partenaire (60,7 %) et de maintenir l’harmonie du couple (57,1 %). Cependant, la fréquence hebdomadaire des rapports sexuels avait diminué dans 75 % des couples. Cette diminution s’expliquait par le temps consacré au nourrisson (66,7 %) et par les dyspareunies (57,1 %). Nous avons aussi observé une fréquence plus importante du coït anal dans le post-partum par rapport à la période d’avant l’accouchement (17,8 % vs 3,57 %). De même, l’usage d’artifices tels que les lubrifiants était plus fréquemment observé dans le post-partum (39,3 % vs 3,6 %). Soixante quatre virgule trois pour cent des patientes avaient trouvé les rapports sexuels moins satisfaisants. Les dyspareunies étaient plus fréquentes en cas d’épisiotomie et de déchirure vulvo-périnéale lors de l’accouchement. Les patientes étaient dans 51 % des cas elles-mêmes responsables du refus de la reprise de l’activité sexuelle. Les raisons du refus étaient d’ordre culturel et religieux et en rapport avec l’absence du retour des couches dans respectivement 64,7 % et 54,9 % des cas.
Conclusion |
L’accouchement a eu un impact négatif sur la reprise l’activité sexuelle des couples. De même, nous avons noté une forte influence des croyances culturelles et religieuses sur la pratique sexuelle du post-partum en l’absence d’informations fiables. Une attention particulière devrait être portée par le corps médical sur la question de la sexualité dans le post-partum.

Patients and method |
It was about study type attitudes and practical knowledge which took place in the department of gynecology-obstetrics and andrology of the teaching hospital of Bouaké over the period going in September 30th, 2012 on April 30th, 2013. The study took place in two stages. The first stage had consisted in a pre-test which allowed validate the questionnaire which contained 16 items. The second stage consisted submitting it directly to the patients, in postnatal consultation, who had agreed to participate in the study after an informed consent. The data were analyzed with epi-info, 2002 version.
Results |
On 395 patients investigated, we noted the resumption of the sexual intercourse for 140 patients (34,2 %). The resumption was made on the initiative of the partner in 67,9 % of the cases, in particular after the return of menstruation (53,8 %). The reason to accept sexual intercourse was to satisfy the partner (60,7 %) and to maintain the harmony of the couple (57,1 %). However, the weekly frequency of the sexual intercourse had decreased in 75 % of the couples. This decrease was caused by the time dedicated to the child (66,7 %) and by the dyspareunia (57,1 %). We also observed a more important frequency of the anal sexual intercourse in the post-partum compared with the period before the childbirth (17,8 % vs. 3,57 %). Also, the use of subtleties, such as lubricants was observed more frequently in the post-partum (39,3 % vs. 3,6 %). Sixty-four comma three percent of the patients had found less satisfaction sexual intercourse. Dyspareunia was more frequent in case of episiotomy, perineal tear during the delivery and delivery assisted (vacuum). The patients, in 51 % of the cases, were themselves responsible for the refusal of the resumption of the sexual activity. The reasons of the refusal were of cultural and religious order and in touch with the absence of the return of menstruation in respectively 64,7 % and 54,9 % of the cases.
Conclusion |
The delivery had a negative impact on the resumption of couple sexual activity. Also, we noted a strong influence of the cultural and religious faiths on the sexual practice of the post-partum in the absence of reliable information. A particular attention should be concerned by the medical profession on the question of sexuality in the post-partum.
